동남아시아 결혼이주여성 임부의 생활스트레스, 사회적 지지 및 산전우울과의 관계

Abstract
우리나라 2013년 통계청 자료에 의하면 동남아시아 결혼이주여성 수는 55,771명이며, 베트남, 필리핀, 캄보디아 여성이 주류를 이루고 있다. 이들은 타국에서의 새로운 삶에 적응하기도 전에 임신과 출산이라는 중대사건을 겪으면서 이중의 스트레스를 겪고 있다. 본 연구는 동남아시아 결혼이주여성 임부의 생활스트레스, 사회적 지지 및 산전우울 실태와 그 관계를 조사하고 산전우울 예측요인을 분석하기 위한 서술적 조사연구이며, 이를 바탕으로 향후 동남아시아 결혼이주여성 임부들을 위한 간호중재 프로그램 개발의 기초자료를 제공하기 위함이다. 연구대상자는 한국인 남성과 결혼하여 D광역시에 거주하는 동남아시아 여성(베트남, 필리핀, 캄보디아)으로서 임신진단을 받고 다문화가족지원센터, 여성전문병원에서 산전관리를 받는 93명의 임부를 대상으로 하였다. 연구도구는 생활스트레스, 사회적 지지와 산전우울(K-EPDS), 사회적 지지 관련 특성으로 이루어졌다. 설문지는 해당국가의 언어로 번역하고 공증 받은 설문지를 제공하였고, 자료분석은 SPSS version 18.0 통계프로그램을 이용하여 빈도분석, t-test, ANOVA, 상관관계분석, 단계적 다중회귀분석을 사용하였고, 본 연구의 연구결과를 요약하면 다음과 같다. 1. 대상자의 생활스트레스는 평균 2.65점(5점 만점)으로 나타났으며, 사회적 지지는 평균 3.17점(5점 만점)으로 나타났고, 산전우울은 평균 10.16점(0-30점)이었으며, 대상자의 49.5%가 정상(0-9점), 17.2%가 우울 경계선(10-12점), 33.3%가 심각수준(13점 이상)인 것으로 나타났다. 2. 대상자의 특성에 따른 생활스트레스 차이에서 통계적으로 유의미한 차이가 나타난 변수들은 월수입(p=.020), 직장((p=.015), 결혼생활만족도(p=.006), 지원정책인지(p=.020)이었으며, 사회적 지지에서는 결혼생활만족도(p=.047), 동거가족(p=.043), 지원정책 인지경로(p=.016)이었고, 산전우울에서는 월수입(p=.020), 결혼생활만족도(p<.001), 지원정책 인지경로(p=.049), 지원정책만족도(p=.033)이었다. 3. 생활스트레스와 산전우울은 양의 상관관계가 있었고(r=.59, p<.001), 생활스트레스(r=-.42, p<.001)와 산전우울(r=-.39, p<.001)은 사회적 지지와 각각 음의 상관관계가 있었다. 4. 대상자의 산전우울 예측요인을 분석한 결과 생활스트레스(ß=.45), 결혼생활만족도(ß=-.22), 사회적 지지 중 정서적 지지(ß=-.18)순으로 나타났으며, 이러한 예측요인들은 산전우울을 43% 설명하는 것으로 나타났다. 결론적으로 대상자의 생활스트레스가 낮을수록, 결혼생활만족도가 높을수록, 사회적 지지 중 정서적 지지가 높을수록 산전우울이 낮아지는 것으로 나타났다. 따라서 산전우울을 감소시키기 위해서는 이들이 처한 상황과 문화적 측면을 고려한 세분화된 산전우울 예방 간호중재 프로그램의 개발이 필요하며, 산전 진료시 스크리닝을 통하여 산전우울 가능성이 있는 결혼이주여성들을 선별하여 조기 진단, 조기 중재를 하도록 노력해야 할 것이다.

The purpose of this descriptive study was to investigate life stress, social supports, and antepartum depression among pregnant Southeast Asian women in Korea. The participants were 93 pregnant, Southeast Asian women (from Vietnam, Cambodia, and the Philippines), married to Korean men and residing in D city, South Korea. Subjects were recruited from local women's clinics and multicultural family support centers. The instruments used were life stress scales developed by Shin (1992) and modified by the researcher, social support scales developed by Shin (1992), and K-EPDS translated by Han, Kim, and Park (2004). All instruments were translated and notarized into their corresponding mother languages (Vietnamese, Cambodian, and Tagalog). The data was analyzed using SPSS/WIN (Version 18.0). Results of the study were as follows: 1. The average scores of life stress were 2.65 (0.50) on a scale of 1-5, social support was 3.17 (0.34) on a scale of 1-5, and antepartum depression was 10.16 on a scale of 0-30. 17.2% was mild (10-12), and 33.3% was severe (>13). 2. Life stress was significantly different among variables such as monthly income (F=3.44, p=.020), employment (t=2.47, p=.015), marital satisfaction (F=5.37, p=.006), and knowledge about government support policy (t=-2.38, p=.020). 3. As for social support, the data collected is as follows: marital satisfaction (F=3.15, p=.047), number of family members living together (F=2.58, p=.043) and source of information about government support policy (F=4.53, p=.016). 4. Antepartum depression was significantly different according to monthly income (F=3.46, p=.020), marital satisfaction (F=9.84, p<.001), source of information about government support policy (F=3.22, p=.049) and satisfaction about the government support policy (t=-2.20, p=.033). 5. There was a positive correlation between life stress and antepartum depression (r=.59, p<.001). Social support was negatively correlated with life stress (r=-.42, p<.001) and antepartum depression (r=-.39, p<.001). 6. The best predictors for antepartum depression were life stress (ß=.45), marital satisfaction (ß=-.22), and emotional support (ß=-.18) and predicted 43% of variance in antepartum depression. In conclusion, the scores of antepartum depression were positively correlated with the scores of life stress and negatively correlated with marital satisfaction and emotional support. Accordingly, in order to prevent antepartum depression among pregnant women from Southeast Asia, culturally sensitive antepartum nursing intervention may be needed. Particularly, antepartum screening and counseling may be necessary for potentially vulnerable groups such as pregnant women of Southeast Asian immigrants married to Korean men.
