Abstract
The aim of this study was to develop and validate a prediction of postoperative delirium (POD) scoring system to identify individuals with a high probability of postoperative delirium at the end of surgery on the basis of preoperative and immediate postoperative data. The study design was a prospective, observational study. Development of the POD scoring system after major surgery was conducted from June 2013 to January 2014 and included 561 patients. Validation of the POD scoring system after major surgery was conducted from February 2014 to October 2014 and included 553 patients. Data was analyzed by descriptive statistics, Chi-square, Fisher's exact test, Mann Whitney U-test, Logistic regression, ROC (receiver operation characteristic), AUC (area under the curve) and Validation (Sensitivity, Specificity, Positive predictive value, Negative predictive value, Accuracy) using IBM SPSS 20.0. The major results of this study were as follows: 1) The risk factors for postoperative delirium were identified as the following: old age (p=.001), previous history of delirium (p=.010), low physical activity (p<.001), hearing difficulty (p=.030), heavy alcoholism (p=.006), intensive care units admissions (p<.001), open surgery or vascular surgery (p=.001), preoperative C-reactive protein ≥ 10.0㎎/㎗ (p=.048), and emergency surgery (p=.032). 2) The POD scoring system including the 9 risk factors was developed. The maximum score was 16 and a high risk of postoperative delirium was defined as a score of more than 7 (AUC=.911, p<.001). 3) Validation of the POD scoring system was sensitivity 83.8%, specificity 92.5%, positive predictive value 70.9%, negative predictive value 96.3% and accuracy 91.0% (AUC=.943, p<.001). The POD scoring system is a simple, predictive model based on 9 risk factors. Therefore, use of the POD scoring system could help to prevent or mitigate postoperative delirium after major surgery for patients with high delirium risk.
본 연구는 외과 수술환자를 대상으로 수술 후 섬망에 영향을 미치는 요인을 파악하여, 수술 후 섬망 예측도구를 개발하고, 도구의 타당성을 검증하기 위한 목적으로 실시되었다. 연구설계는 도구 개념틀 구성, 체계적 문헌고찰, 내용타당도 검증, 구성타당도 검증, 외적타당도 검증의 다섯 단계를 전향적인 방법으로 시행하였다. 도구개발은 2013년 6월부터 2014년 1월까지 주요 외과수술을 받은 60세 이상 환자 561명을 대상으로 하였고, 도구검증은 2014년 2월부터 2014년 10월까지 주요 외과수술을 받은 60세 이상 환자 553명을 대상으로 하였다. 자료분석은 IBM SPSS 20.0 Program을 이용하여, 섬망 발생 유무에 영향을 미치는 위험요인과 도구개발은 Chi-square test와 Fisher’s exact test, Mann-Whitney U-test, 이분형 로지스틱 회귀분석을 사용하였으며, 개발된 도구의 섬망 예측점수와 예측정확도는 ROC (receiver operation characteristic)의 AUC (area under the curve), 도구검증은 민감도, 특이도, 양성예측도, 음성예측도, 정확도 검증을 실시하였다. 연구결과는 다음과 같다. 1) 수술 후 섬망 발생에 영향을 미치는 위험요인은 나이(p=.001), 섬망력(p=.010), 수술 전 낮은 활동상태(p<.001), 청력장애(p=.030), 심한 음주력(p=.006), 중환자실 입실(p<.001), 복부 및 혈관 절개수술(p=.001), 수술 전 높은 CRP (p=.048), 응급수술(p=.032)로 총 9개였다. 2) 수술 후 섬망 예측도구는 총 16점으로 구성되었으며, 7점 이상인 경우 섬망 위험군으로 분류되었다(AUC=.911, p<.001). 3) 수술 후 섬망 예측도구의 타당도는 민감도 83.8%, 특이도 92.5%, 양성예측도 70.9%, 음성예측도 96.3%, 정확도 91.0%이었다(AUC=.943, p<.001). 이상의 결과로 수술 후 섬망 예측도구는 모두 9가지 위험요인으로 구성되었고, 높은 타당도를 나타내었다. 이에 임상에서 수술 후 섬망 예측도구를 사용하여 섬망 예방중재활동이 필요한 위험군을 간편하고 효율적인 방법으로 선별할 수 있을 것이다.
