방사선요법을 받는 여성암 환자의 수면의 질과 영향요인

Abstract
The purpose of this study was to investigate the subjective sleep quality and factors related to poor sleep quality in breast and gynecologic cancer patients undergoing radiation therapy.
Data were collected between July 23, 2012, and February 27, 2013, from 94 cancer patients (68 breast cancer patients and 26 gynecologic cancer patients) who were being treated in one general hospital in Daegu. Subjective sleep quality was measured by the Pittsburgh Sleep Quality Index (PSQI), the Revised Piper Fatigue Scale (R-PFS) was used to measure subjective and multidimensional fatigue, and the Hospital Anxiety Depression Scale (HADS) was used to measure subjective anxiety and depression. Data were analyzed through SPSS WIN 20.0 for frequency, percentage, means, standard deviation, t-test, chi-square test, Fisher's exact test, and multiple logistic regression.
The results of the study were as follows. The average global PSQI was 8.34 and 70% of the patients in this study reported poor sleep quality. Concurrent chemoradiotherapy (p=.022), the number of radiation treatments (p=.013), total amount of radiation dose (p=.048), and taking sleeping pills (p=.030) were the significant differences between the poor sleep quality and good sleep quality groups. Fatigue (p=<.001), anxiety (p=<.001), and depression level (p=.024) were the significant differences between the poor sleep quality and good sleep quality groups. Multiple logistic regression showed that significant predictors for poor sleep quality were fatigue, and concurrent chemoradiotherapy.
The study findings suggest that breast and gynecologic cancer patients undergoing radiation therapy require special concern regarding the risk of poor sleep quality. Regular sleep quality screening could be beneficial for early detection of poor sleep quality and effective nursing strategies should be considered to improve sleep quality among breast and gynecologic patients undergoing radiation therapy.
본 연구는 방사선요법을 받는 여성암 환자의 수면의 질을 확인하고, 수면의 질에 영향을 미치는 요인을 파악하고자 실시하였다. 자료수집은 2012년 7월 23일부터 2013년 2월 27일까지 D시 A대학교병원에서 방사선요법을 받는 유방암 환자 68명과 부인암 환자 26명, 총 94명을 대상으로 하였다. 수면의 질은 Pittsburgh Sleep Quality Index (PSQI)를 이용하여 측정하였고, 피로는 Revised Piper Fatigue Scale (R-PFS), 불안 및 우울은 Hospital Anxiety and Depression Scale (HADS)을 이용하여 측정하였다. 자료의 분석은 SPSS WIN 20.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, 카이제곱 검정, Fisher's exact test, 다중 로지스틱 회귀분석(Multiple logistic regression)으로 분석하였다.
연구결과 방사선요법을 받는 여성암 환자의 수면의 질은 8.34점으로 전체 대상자의 70%가 수면의 질이 낮은 것으로 나타났다. 수면의 질이 낮은 군은 수면의 질이 양호한 군보다 방사선요법과 항암화학요법을 병행하고 있는 경우(p=.022)가 많았고, 방사선 총 조사 횟수(p=.013)와 방사선 총 조사량(p=.048)에서 유의한 차이가 있었으며, 수면제를 복용하는 빈도(p=.030)가 높았다. 수면의 질이 낮은 군은 수면의 질이 양호한 군보다 피로(p=<.001)가 심하고, 불안(p=<.001) 및 우울(p=.024)이 높은 것으로 나타났다. 방사선요법을 받는 여성암 환자의 수면의 질에 유의하게 영향을 미치는 요인은 피로와 방사선요법과 항암화학요법의 병행으로 나타났다.
결론적으로 방사선요법을 받는 여성암 환자의 상당수가 수면의 질이 낮음을 알 수 있다. 따라서 방사선요법을 받는 여성암 환자의 수면의 질에 대한 특별한 관심이 필요하며, 정기적인 수면의 질 파악은 수면장애를 조기에 발견하는데 도움이 될 것이다. 이 연구결과를 바탕으로 방사선요법을 받는 여성암 환자의 수면의 질을 증진시키는 효과적인 간호전략 개발이 필요하다.
