노인환자의 심폐소생술 금지 결정에 대한 간호사의 인식과 정서상태

Abstract
This research is a descriptive survey research that provides basic data to develop a nursing guideline that includes the DNR decision making process, the role of the nurses after the decision is made, enhancement plan of the coping skills and maintenance of dignity of the end of life geriatric patient as a human being by grasping the recognition and the emotional state of the nurse after DNR is prescribed to the geriatric patient. Out of four general hospitals in D metropolitan city which DNR situation is frequently occurring, such as intensive care unit, emergency room, medical ward, and surgical ward, the subject of study were 229 nurses who were in charge of geriatric patients older than 65 who were prescribed DNR. Instruments for experience and recognition of DNR geriatric patients, and the Korean profile of mood state machine (K-POMS) were used and χ2-test, t-test and ANOVA were used for data analysis.
The study of this research is as follows:
1. 65.9% of the decider of DNR was their son and only 1.3% of the decider was the patients, themselves. 51.5% of the nurses received the consultation request and 28.4% had received education on the DNR.
2. 94.3% of the nurses responded that the DNR is necessary. The 55.0% of response for the most of the main reason for this was for ‘the comfortable and dignified death.’ Of the geriatric patients DNR decision, the respond showed that 75.2% answered that the desirable decider is the ‘will of the patient and the patients family’ and 89.1% of the nurses responded that DNR guideline establishment is necessary.
3. Out of the emotional state items, ‘I’m not worried’showed the highest score and this is was a reverse question which means that they are actually worried about DNR decision. The next highest to lowest ordered items were ‘I cannot do anything with my strength’, ‘sad’, ‘have relieved feeling’ and ‘depressed.’
4. There was statistically significant change on the nurses age, existence and nonexistence of desire of DNR operation to their own family member based on their religions, the religion of the nurses, existence and nonexistence of desire of DNR operation to themselves based on clinical work experience, material status and religion, and the emotional state of the nurse after the DNR decision based on the clinical work experience. There was statistically significant difference on the necessity of DNR, the right time for the DNR explanation, and the emotional state of the nurses about the DNR operation.
Concluding the stated above research, autonomy of the patients are excluded during the DNR decision process. Most of the nurses are dealing with the consultation without receiving the systemic education on DNR and after the DNR decision is made, they experience anxiety, helplessness, sadness, depression and so on.
Therefore, systems, that make clear patients’thought on DNR in document based on adequate understanding on their illness, soon need to be fixed. Also, systemic and persistent education on DNR during undergraduate courses or during practical affairs is needed and the nursing guideline that includes the DNR decision making process, the role of nurses and the enhancement plan of their coping skills is necessary.
본 연구는 노인환자의 DNR 결정에 대한 간호사의 인식과 정서 상태를 파악하여 생의 말기에 있는 노인환자가 인간으로서의 존엄성을 유지하도록 돕고, DNR 결정과정과 결정 후 간호사의 역할과 대처능력 향상 방안을 담은 간호실무 지침 개발을 위한 기초자료를 제공하고자 시도된 서술적 조사 연구이다. D광역시에 소재하고 있는 4개의 종합병원의 DNR 상황이 자주 발생하는 중환자실, 응급실, 내과병동, 외과병동에서 근무하는 간호사 중 DNR 결정이 내려진 65세 이상의 노인환자를 담당한 간호사 229명을 대상으로 하였다. 노인환자 DNR에 대한 경험 및 인식에 대한 도구와 한국판 기분상태 측정도구(K-POMS)를 사용하였으며, 자료 분석은 χ²-test, t-test와 ANOVA를 사용하였다.
본 연구의 결과는 다음과 같다.
1. DNR에 대한 주의사결정자는 아들이 65.9%로 가장 많았으며, 환자 본인이 결정한 경우는 1.3%였다. 51.1%의 간호사가 DNR과 관련하여 환자나 보호자로부터 상담 요청을 받은 경험이 있었고, DNR에 대해 교육을 받은 경험이 있는 간호사는 28.4%였다
2. DNR이 필요하다고 응답한 간호사는 94.3%였으며, DNR이 필요한 주된 이유로 ‘편안하고 품위 있는 죽음을 위해’라는 응답이 55.0%로 가장 많았다. 노인환자의 DNR 결정에 있어 바람직한 결정자로는 75.2%가 ‘환자와 가족의 의지’라고 응답하였으며, DNR 지침서 제정의 필요성에서는 89.1%의 간호사가 필요하다고 응답하였다.
3. 정서 상태 항목 중 ‘걱정이 없다’는 가장 높은 점수를 보였으며 이는 역 문항 처리한 것으로 DNR 결정과 관련하여 걱정이 된다는 것을 의미한다. 다음으로 점수가 높은 항목은 ‘내 힘으로는 어쩔 수 없다’, ‘슬프다’, ‘안도감이 든다’, ‘우울하다’ 순이었다.
4. 간호사의 연령, 종교에 따른 간호사 본인의 가족에 대한 DNR 시행 희망 유무, 간호사의 종교, 임상근무경력에 따른 간호사 본인에 대한 DNR 시행 희망유무 항목에서 통계적으로 유의한 차이가 있었다.
간호사의 결혼 상태, 종교, 임상근무 경력에 따른 정서상태에서 통계적으로 유의한 차이를 보였으며, DNR의 필요성, DNR 설명의 적절한 시기, 간호사 본인의 DNR 시행희망유무에 대한 정서 상태에서도 통계적으로 유의한 차이가 있었다.
이상의 연구를 종합해 볼 때 DNR 결정과정에서 환자의 자율성은 배제되고 간호사는 DNR에 대한 체계적인 교육을 받지 못한 상태에서 상담에 임하고 있으며, DNR 결정에 대해 걱정, 무력감, 슬픔 등을 경험하고 있다. 그러므로 본인의 병 상태에 대한 충분한 이해를 바탕으로 자신의 생각을 미리 서면으로 밝혀두는 제도가 조속히 정착되어야 할 것이며, 학부과정이나 실무에서 DNR에 대한 체계적이고 지속적인 교육과 DNR 결정과정, 결정 후 간호사의 역할과 대처능력 향상 방안이 포함된 간호실무 지침이 마련되어야 할 것이다.
