신이식자의 거대세포바이러스 위염의 내시경적 특징

Abstract

The endoscopic characteristics of gastric cytomegalovirus (CMV) infection are various and mimic other gastric diseases. The aim of this study was to find the endoscopic features associated with gastric CMV infection. Data 301 from renal transplant patients (RTPs) who underwent endoscopic examination with mucosal biopsies for CMV infection between March 2005 and July 2013 were reviewed retrospectively. Gastric CMV infection was confirmed by immunohistochemical staining and CMV polymerase chain reaction. The endoscopic characteristics of ulcers and erosions, such as number, size, morphology, location, and exudates, were evaluated for the potential risk factors of CMV infection. For ulcer lesions, round shape and antral location were significantly associated with CMV infection. For erosions, exudates attachment, raised type, and antral location were more significant in the CMV infection group. These endoscopic characteristics of ulcers and erosions may be helpful to predict gastric CMV infection in patients with renal transplantation. Further study is needed to validate the results of this study.

신장 이식 후 신이식자의 예후에 영향을 주는 거대세포바이러스 감염은 매우 흔하게 발생된다. 이 연구의 목적은 거대세포바이러스 감염을 예측하는데 도움이 될 수 있는 내시경적 특징을 파악하는 것이다. 2005 년 3 월부터 2013 년 7 월 사이 계명대학교 동산의료원에서 신장 이식을 받은 301 명의 환자를 대상으로 위내시경 검사를 하였고 이를 후향적으로 분석하였다. 거대세포바이러스 감염의 유무를 확인하기 위해 조직검사, 면역조직화학적 염색법 및 거대세포바이러스 중합효소연쇄반응법을 시행하였다. 내시경 기록을 참조하여 궤양과 미란성 위염의 수, 크기, 형태, 삼출물의 유무, 위치를 분류하였다. 60 명의 궤양 군에서 궤양의 병변이 전정부에 위치하거나 궤양의 형태가 원형일 때에 거대세포바이러스 감염이 많은 것으로 확인되었으며 185 명의 미란성 위염 군에서는 병변이 전정부에 위치하거나, 융기 형태 또는 삼출물이 동반 되었을 때 거대세포바이러스 감염이 많이 확인 되었다. 원형의 궤양성 병변과 융기 형태의 삼출물이 동반된 미란성 위염이 전정부에 같이 있는 경우에는 모두 거대세포바이러스 감염이 확인 되었다. 이러한 궤양과 미란성 위염의 내시경적 특징들은 신이식 환자의 거대세포바이러스 위염을 진단하는 예측인자로 도움이 될 것이라 생각된다. 그러나 이 연구의 결과를 보다 검증하기 위해서는 추가적인 연구가 필요하다.
