Hepatic Resection and Prognosis for Patients with Single Hepatocellular Carcinoma Larger than 5

Abstract
This study aimed to determine the clinicopathological factors that influence the prognosis of hepatic resection for HCC larger than 5 cm. Of 421 patients surgically resected HCCs, One hundred-fourteen patients (27.1%) had single HCC larger than 5 cm. Clinical features and factors influencing the outcome of these patients were retrospectively analyzed. The disease free survival and overall survival rate at 5 years was 29% and 43%. Multivariate analysis showed that the presence of microvascular invasion or macrovascular invasion, positive hepatitis B, and elevated PIVKA-II were the independent factor of worse outcome of disease free-survival rates in single HCC larger than 5 cm. In conclusion, hepatic resection is a feasible treatment for single HCC larger than 5 cm. Long term outcome was significantly influenced by the presence of micro- or macrovascular invasion, positive hepatitis B, and elevated PIVKA-II.
