지난 10년간 일 대학병원 정신건강의학과 소아청소년 초진환자 분포의 변화

Abstract

대구지역 일 대학병원 정신건강의학과를 방문한 소아청소년 초진환자를 대상으로 연령, 성별 및 진단적 분포를 조사하였다. 그리고 그 변화 추이를 분석하고 이전 연구 결과들과 비교하여 향후 소아청소년 정신과 연구의 기초자료를 마련하고자 하는 것이다. 2004년 1월 1일부터 2013년 12월 31일까지 10년간 계명대학교 동산의료원 정신건강의학과에 내원한 만 18세 이하의 초진환자 2,738명(남자 1,906명, 여자 832명)을 대상으로 하여 이들의 의무기록을 조사 분석하였다. 2004년부터 2013년까지 환자 군을 분석하면 남녀의 비율은 2.3 : 1이었으며, 7 - 9세 사이의 환자가 가장 많았다. 평균연령은 남자는 9.84 ± 4.59세, 여자는 10.76 ± 4.81세, 전체 연령은 10.12 ± 4.68세였다. 전체 외래 환자에 대한 소아청소년 환자의 비율은 27.0%였다. 전체 대상의 질환 군 분포는 ADHD, 정신지체, 의사소통장애 군의 순이었다. 성별에 따른 질환 군 분포는 남자에서는 ADHD, 정신지체, 의사소통장애 군의 순으로 높은 빈도를 보인 반면, 여자에서는 정신지체, ADHD, 우울장애 군의 순으로 나타났다. 연도별 질환 군은 2010년, 2013년을 제외하고는 ADHD 군이 해마다 가장 많았고 다음으로 정신지체 군의 순이었다. 결론적으로 여자 환자의 비율과 전체 평균연령이 점차 증가하는 추세였다. ADHD 군이 가장 많이 진단되었고 우울장애 군이 점차 증가하는 추세를 보였다.

The purpose of this study was to investigate changes in child and adolescent outpatients at a university hospital in Daegu from 2004 to 2013, and to compare these to prior studies. The subjects were first-visit patients under 18 years old, who visited Dongsan Medical Center, Keimyung University, from January 1, 2004, to December 1, 2013. There were a total of 2,738 patients (male 1,906, female 832) whose computerized medical records were investigated. The ratio of male to female was 2.3 : 1 in the period from 2004 to 2013. The most prevalent age category was 7 - 9 years. The mean age was 10.12 ± 4.68 years in total, and (9.84 ± 4.59 years in males, 10.76 ± 4.81 years in females). The ratio of child and adolescent patients to total outpatients was 27.0%. The most common diagnostic category was the ADHD group, followed by the mental retardation(MR) group and the communication disorder group were next. In males, the ADHD, MR, communication disorder groups were more prevalent, but in females, the MR, ADHD, and depressive disorder groups were more prevalent. In conclusion, the female to male ratio and total mean age were on the rise. The ADHD group was the most prevalent. The depressive disorder group was also on the rise.
