강직을 동반한 회전근 개 파열에서 관절경하 관절낭 유리술 시행한 회전근 개 봉합술의 임상적 결과

Abstract

This study sought to compared clinical outcomes after surgical treatment between rotator cuff tear with and without shoulder stiffness and evaluated the serial changes in pain, functional scores, and range of motion (ROM). Twenty-six patients with preoperative stiffness (stiff group) and without stiffness (non-stiff group) were included. The stiff group underwent rotator cuff repair with arthroscopic capsular release and manipulation. Visual Analog Scale (VAS) pain score, University of California Los Angeles (UCLA) and American Shoulder and Elbow Surgeons (ASES) scores, and ROM were evaluated serially. There were no significant differences between two groups with regard to VAS pain score, UCLA and ASES scores at any period after surgery. In the stiff group, mean forward flexion was significantly lesser than that in the non-stiff group at 3 months after surgery. Mean external and internal rotation were significantly lesser than those in the non-stiff group at 3 and 6 months after surgery. Overall satisfactory outcomes could be achieved in both groups, although the stiff group had slower postoperative recovery of ROM up to 6 months after surgery.

강직의 동반 유무에 따른 회전근 개 파열의 수술적 치료 후 임상적인 결과를 비교하고, 통증, 기능상의 점수 및 관절 운동 범위의 연속적 변화를 평가하였다. 술 전 강직을 동반한 26예와 술 전 강직을 동반하지 않은 26예를 비교 분석하였으며, 강직을 동반한 군에서 관절경하 관절낭 유리술과 도수조작술을 병용한 회전근 개 봉합술을 시행하였다. VAS 통증 점수, UCLA 점수, ASES 점수, 관절 운동 범위 등을 평가하였다. 두 군 모두 최종 추시시 VAS 통증 점수, UCLA 점수, ASES 점수, 관절 운동 범위의 호전을 보였다. 술 후 모든 기간에서 VAS 통증 점수, UCLA 점수, ASES 점수, 관절 운동 범위는 두 군간에 유의한 차이를 보이지 않았다. 강직을 동반한 군에서 술 후 3개월 째 전방 굴곡의 각도가 적었으며, 술 후 3개월, 6개월 째 외회전 및 내회전의 각도가 적었다. 두 군 모두 만족스러운 임상적인 결과를 보여 주었지만, 강직을 동반한 군에서 술 후 6개월까지 관절 운동 범위의 회복이 느림을 알 수 있었다.
