한국인 강직성 척추염 환자의 수면의 질과 질병 활성도의 관계

Abstract

강직성 척추염(ankylosing spondylitis, AS)은 혈청인자음성 척추관절염(seronegative spondyloarthritis, SNSA) 중 하나로 주로 축골격(axial skeleton)에 호발하는 만성 염증성 질환이다. 강직성 척추염은 관절통증, 조조강직과 같은 증상으로 인해 수면의 질적 저하를 유발할 수 있다. 본 연구는 한국인 강직성 척추염 환자를 대상으로 수면의 질을 정상 대조군과 비교하고 질병 활성도와 수면의 질과의 관계를 파악 하고자 하였다. 강직성 척추염 환자 35명과 다른 질환이 없는 정상 대조군 100명이 연구에 참여하여 수면의 질은 설문조사를 통해 한국판 피츠버그 수면의 질 지수(Korean version of the Pittsburgh Sleep Quality Index, PSQI)를 측정하였고 총 PSQI 점수가 5점을 초과하면 수면장애가 있는 군(poor sleeper)으로 정의하였다. 환자군은 Bath 강직성 척추염 질환 활동성 지수(Bath ankylosing spondylitis disease activity index, BASDAI)와 강직성 척추염 질병 활성도 점수(Ankylosing spondylitis disease activity score, ASDAS)를 측정하여 질병 활성도를 평가하였다. 강직성 척추염 환자중 54.3%가 수면장애가 있는 군으로 분류되었고 이는 대조군 14%에 비해 유의하게 높았다. ASDAS-CRP를 기준으로 질병 활성도가 높을수록 총 PSQI점수가 유의하게 높았다. 또한 조조강직 지속시간과 우울증이 수면장애에 영향을 미치는 독립적 위험인자였다. 결론적으로 강직성 척추염 환자에서 수면의 질을 개선하기 위해 질병 활성도와 우울증상을 적극적으로 조절하는 것이 중요하겠다.

Sleep disturbance is often reported in patients with ankylosing spondylitis (AS). This study was designed to evaluate association with sleep quality and disease activity of Korean AS patients. 35 patients with AS and 100 healthy controls were included in this study. Sleep quality was assessed using the Korean version of the Pittsburgh sleep quality index (PSQI). Depression was assessed by the Korean version of the Beck depression inventory second edition (BDI-2). The Bath ankylosing spondylitis disease activity index (BASDAI) and ankylosing spondylitis disease activity score-c-reactive protein (ASDAS-CRP) were used to evaluate disease activity. The total PSQI score was higher in the AS patients than in the control subjects. Sleep disturbance was found in 54.3% of patients. High disease activity and depression were observed in patients with sleep disturbances. Significantly, higher disease activity was associated with poor sleep quality and depression. Stepwise multiple regression analysis revealed that morning stiffness duration and depression were independent risk factors that influenced poor sleep quality. Further study is required in the evaluation and management of disease activity and depression to improve sleep quality in Korean AS patients.
