염증성장질환 및 자가면역성간염에서 Azathioprine 치료의 부작용 양상 차이

Abstract

Azathioprine(AZA)/6-mercaptopurine(6-MP)은 생체 내에서 핵산 대사를 방해함으로써 세포증식과 면역억제 기능을 나타내는 약물들로 염증성장질환 및 자가면역성간염의 치료에 광범위하게 사용되고 있다. 본 연구에서는 2000년 1월 1일부터 2011년 3월 31일까지 계명 대학교 동산 의료원 소화기 내과에서 AZA/6-MP를 처방 하였던 염증성장질환 환자 및 자가면역성간염 환자의 의무 기록을 후향 분석하여 AZA/6-MP와 연관된 부작용의 빈도와 발생 시기 및 그 기전에 대하여 알아보았다. 또한 부작용에 관계되는 TPMT 대립유전자 다형성(polymorphism)을 조사하고자 하였다. 계명대학교 동산의료원에서 AZA를 복용하였던 139명의 염증성장질환 환자들과 55명의 자가면역성간염 환자들이 최종 등록 되었다. 투여 기간 중 염증성장질환 환자 30명(21.6%)에서 37건의 부작용이 발생하였으며 55명의 자가면역성간염 환자 8명(14.5%)에서 부작용이 발생하였다. 두 군에서 동일 용량의 AZA을 복용함에도 염증성장질환 군에서의 백혈구 감소증의 비알러지 반응이 유의하게 높았다. 총 10명의 검사 대상자 가운데 7명의 TPMT 유전자형은 TPMT*1으로 조사되었고, 3명은 TPMT*1S를 나타내어 골수기능 억제와 TPMT 유전자형의 변이가 불일치하는 결과를 나타내었다. 이는 질환에 따른 골수기능 억제의 다른 빈도가 모두 TPMT 유전형 다형성에 기인하는 것은 아닌 것임을 시사한다. 이들 결과들을 바탕으로 추후 좀 더 대규모의 전향적인 연구를 통해 소화기 질환에서 AZA/6-MP 치료적 관점의 가이드 라인이 도출될 것이라고 생각된다.

Immunomodulation with azathioprine(AZA) has been used in the maintenance therapy of inflammatory bowel disease(IBD) and autoimmune hepatitis(AIH). However, there is a lack of research evaluating the adverse effects of AZA on the two groups. This study aimed to compare the adverse reaction of AZA on the two groups. IBD and AIH patients who received thiopurine treatment at Dongsan Medical Center, between January 2000 and March 2011, were enrolled for this retrospective study. The study compared the clinical characteristics and adverse effects of AZA. In addition, it analyzed the polymorphism TPMT genotype in the two groups. A total of 139 IBD(89M/50F; age 38.71 ± 15.75 yrs) and 55 AIH(12M/43F; age 55.38 ± 13.95 yrs) patients were finally enrolled. 30 of the 139 IBD patients(21.6%) and 8 of 55 AIH patients(14.5%) experienced adverse effects of the drug. In particular, the prevalence of leukopenia was significantly higher in the IBD group compared to the AIH group(p=0.026). Among the 10 patients, T474C polymorphism was noted in 3 patients. No association was noted for polymorphism TPMT in the two groups. Therefore, TPMT genotype or TPMT activity may not be related side effects of AZA in patients treated with thiopurines.
