주요우울증에서 자살경향성 : 공존질환의 영향

Abstract

일반적으로 주요 우울증에 공존질환이 동반될 때 자살률이 증가된다는 보고가 있으나 아직 국내에는 관련된 연구가 부족한 실정이다. 본 연구에서는 주요우울증에서 공존질환이 자살경향성에 미치는 영향에 대해 조사하였다. 본 연구는 2008년 3월부터 2011년 9월까지 대구경북 지역의 3개 대학병원 정신건강의학과를 방문한 총 776명에 대하여 Mini-International Neuropsychiatric Interview-Plus(MINI-Plus)를 실시하여 주요우울증으로 진단된 288명의 환자를 대상으로 하였고 MINI-Plus의 자살경향성 항목의 평가를 이용하였다. 공존질환이 있는 주요우울증 환자 군은 공존질환이 없는 주요우울증 군에 비해 자살경향성과 자살위험도가 유의하게 높았다. 공존질환 중 불안장애가 동반된 군에서는 주요우울증 환자 군에 비해 자살경향성이 유의하게 높았다. 전체 공존질환의 개수가 많을수록 자살경향성이 높게 나타났고 공존질환 개수에 따라 자살위험도의 유의한 차이를 보였으며 공존질환이 3개 이상인 군에서 자살위험도가 가장 높았다. 본 연구의 결과는 주요우울증 환자들에서 공존질환이 있는 경우 공존질환이 없는 주요우울증에 비해 자살경향성 및 자살위험성이 높았고 공존질환의 개수가 많을수록 자살경향성 및 자살위험성이 높음을 시사하였다. 임상현장에서 우울증 환자를 진료할 때, 공존질환이 있는 환자에서는 특히 불안장애 환자에서 자살사고 및 행동에 대해 좀 더 세심하게 평가하고 이에 대한 적극적인 치료적인 개입을 필요로 할 것이다.

Generally, major depressive disorder (MDD) with psychiatric comorbidities is associated with a high suicide rate, however, this has not been extensively studied in Korea. In this study, we aimed to evaluate the suicidality in MDD with psychiatric comorbidities. The study was performed using patients who visited the psychiatric departments of three university hospitals in the Gyeongbuk area from March 2008 to September 2011. In total, 776 subjects were evaluated using the Mini International Neuropsychiatric Interview-Plus (MINI-Plus), and 288 patients with MDD were recruited for the study. Suicidality and suicide risks were evaluated using the MINI-Plus questionnaires. The MDD patients with comorbidities had a significantly higher suicidality and suicide risk than the patients with pure MDD. Suicidality was significantly higher in the "anxiety disorder" group as a comorbidity than in the pure MDD group. As the total number of comorbidities got higher, suicidality increased, and suicide risks had significant differences depending on the total number of comorbidities. These results suggested that MDD patients with comorbidities have a higher suicidality and suicide risk. In clinical settings, comorbid MDD patients should be approached with more active psychiatric interventions in regards to their suicidal thoughts and actions.
