2세 이하 영아의 원발성 방광요관역류 교정술후 99mTc-DMSA Renal Cortical Scan의 효용

Abstract
반복적인 소아의 요로감염에 있어서 99mTc-DMSA renal cortical scan(DMSA scan)의 민감도와 특이도가 우수함은 이미 검증이 되었지만 방광요관역류(vesicoureteral reflux, VUR) 교정 수술후 환아에 대한 평가에 대해서는 그 효용가치가 연구된 적은 없다. 저자들은 DMSA scan이 VUR 교정 수술후 환아에 대한 평가에서도 유용한 방법으로 생각하였으며, 이 연구를 통해 반복적인 요로감염을 일으킨 2세 이전의 영아에서 발견된 VUR을 수술로 교정하고, 교정 수술성적의 평가 및 신반흔의 호전여부를 판단하기 위한 추적 DMSA scan이 가지는 임상적 효용에 대해 알아보고자 하였다.
2008년 3월부터 2010년 9월까지 계명대학교 동산병원과 대구 파티마병원에서 VUR을 진단받고, 2세 이전에 개복 또는 내시경 수술을 시행받은 환아들 중, 6개월 이상의 추적관찰이 가능하였고 교정 수술후 6개월째 추적 배뇨중 방광요관조영술(voiding cystourethrography, VCUG) 및 DMSA scan을 시행한 24명의 환아(양측 9명, 편측 발생아 15명을 포함하여 33 신)를 대상으로 의무기록을 후향적으로 분석하였다. 교정 수술전 VCUG와 DMSA scan는 수술일로부터 평균 4.4 ? 2.6 개월전에 시행하였다. DMSA scan은 요로감염 진단 후 5일 이내에 시행하였으며 1.5-1.9 MBq/kg의 99mTc-DMSA를 정맥투여하고 3시간 뒤에 low-energy, general-purpose collimator를 장착한 E-CAM? (Siemens, Malvern, Pennsylvania, USA) 또는 DST?XLi? (GE, Little Chalfont, UK)를 이용하여 앙와위와 복와위로 전후면상과 좌우후사면상을 얻었다. 교정 수술은 내시경 수술이 19명(24 신)이고 개복수술이 5명(9 신)이었다. 3명의 환아에서 내시경 수술을 받은 후 VUR의 호전이 없어 개복수술을 추가로 시행하였고, 이들은 개복수술로 간주하였다. VCUG에 따른 VUR의 등급은 1985년 International Reflux Study Committee에서 발표한 등급체계를 적용하였으며 VUR이 없는 경우를 grade 0, 역류가 신우까지 도달하지 못하는 경우는 grade I, 역류가 신우까지 도달하지만 요관의 확장이 없는 경우를 grade II, 경미한 요관의 확장과 꼬임이 있는 경우를 grade III, 신장의 집합계의 확장은 동반되어 있지만 신유두의 형태는 보이는 경우를 grade IV, 심한 요관의 확장과 꼬임이 있고 집합계의 확장이 심해 신유두가 보이지 않는 경우를 grade V로 하였다. DMSA scan에서 신반흔의 정도는 정상인 경우 grade 0, 단일 반흔이 있는 경우 grade I, 다발성 반흔이 있지만 신윤곽이 잘 보존된 경우는 grade II, 다발성 반흔이 있으면서 신윤곽의 변형이 동반된 경우를 grade III로 하였다.
수술당시 환아들의 평균 연령은 17.2개월(range 11-24)이었으며 남아가 13명(18신), 여아가 11명(15신)이었다. 평균 추적검사 시점은 술후 6.8 ? 1.0 개월이었고 International Reflux Study Committee의 Grading System(1985)에 따른 술전 평균 역류의 정도는 3.1 ? 1.3, 평균 신반흔 등급은 1.9 ? 1.0, 술후 6개월의 평균 역류의 정도는 0.9 ? 1.3, 평균 신반흔 등급은 1.3 ? 1.2으로 술전과 비교하여 술후 6개월의 역류의 정도는 유의하게 감소하였고, 신반흔의 정도도 유의하게 감소하였다.(p<0.05)
비록 추적검사기간이 6개월로 짧고 연구대상이 24명으로 적었으나 VUR때문에 급성 요로감염증이 발생한 2세이하 영아에서 DMSA scan에 나타난 신반흔은, VUR 교정 수술후 VUR의 호전과 함께 신반흔의 호전이 추적 DMSA scan에 잘 반영되므로, VUR의 교정수술을 평가하는데 있어서 DMSA scan는 중요한 지표가 되리라 생각된다.
향후 열성 요로감염증을 동반한 VUR에서 교정수술을 받은 더 많은 영아를 대상으로 1 년 이상의 추적 검사를 전향적으로 시행하여 DMSA scan의 유용성을 연구하여야 할 것으로 생각하며, 또한 복부 CT와 초음파 검사를 병행하여 각각의 유용성을 비교하는 연구가 필요할 것으로 생각한다.
The 99mtechnetium-dimercaptosuccinic acid renal cortical scan (DMSA scan) has been proved to be superior in sensitivity and specificity to detecting renal scars in children?s repeated urinary tract infection. But there is rare report in the investigation of the utility of DMSA scan for the evaluation of the anti-reflux surgery performed on infants. Currently, the DMSA scan could be a useful method for the postoperative follow-up. This study is to investigate the clinical utility of follow-up DMSA scan, in infants before 2 years of age who had a history of urinary tract infection with vesicoureteral reflux (VUR) and underwent anti-reflux surgery.
This study was performed on 24 infants before 2 years of age who were diagnosed with primary VUR and underwent open or endoscopic anti-reflux surgery between March 2008 and September 2010 at Keimyung University Dongsan Hospital and Daegu Fatima Hospital in Daegu City, South Korea. Preoperative voiding vesicourethrography (VCUG) and DMSA scan were performed 4.4 ? 2.6 months before the surgery. In all cases, the DMSA scan was performed within 5 days after the diagnosis of urinary tract infection. Six months after surgery, VCUG and DMSA scan was done. Out of the 24 cases, 9 were bilaterality and the other 15 were laterality. Three hours after intravenous injection of 1.5-1.9MBq/Kg of 99mTc-DMSA, the postero-anterior and both posterior oblique scans were done by the use of low-energy, general-purpose collimator with E-CAM? (Siemens, Malvern, Pennsylvania, USA) or DST?XLi?(GE, Little Chalfont, UK). Submucosal injection was done on 19 patients (24 renal units) and open surgery on 5 (9 renal units). Three infants, who underwent open surgery after having submucosal injection because of persistent symptoms, were classified as the patients who underwent open surgery. The follow up studies were done on the basis of the days when they underwent open surgery. The grade of VUR followed the grading system established by International Reflux Study Committee (1985); 'grade 0' as no reflux, 'grade I' as reflux does not reach the renal pelvis; varying degrees of ureteral dilatation, 'grade II' as reflux reaches the renal pelvis; no dilatation of the collecting system; normal fornices, 'grade III' as mild or moderate dilatation of the ureter, with or without kinking; moderate dilatation of the collecting system; normal or minimally deformed fornices, 'grade IV' as moderate dilatation of the ureter with or without kinking; moderate dilatation of the collecting system; blunt fornices, but impressions of papillae still visible, and 'grade V' as gross dilatation and kinking of the ureter, marked dilatation of the collecting system; papillary impressions no longer visible; intraparenchymal reflux. The grading of renal scars is as follows: ?grade 0? as normal findings, a single, peripheral defect on DMSA scan as ?grade I?, ?grade II? as more than two defects with well preserved renal outline, and ?grade III? as multiple defects with contracted kidney.
The mean age of patients was 17.2 months (range 11?24). Male and female were 13 (18 renal units) and 11 (15 renal units), respectively. The follow-up was done 6.8 ? 1.0 months after the anti-reflux surgery. Preoperative VUR grade on VCUG and renal scar grade on DMSA scan were 3.1 ? 1.3 and 1.9 ? 1.0, respectively. Six months after the surgery, they showed VUR grade 0.9 ? 1.3 and renal scar grade 1.3 ? 1.2. As a result, follow up DMSA scan shows significantly lowered grade of renal scar comparison with 6 months before (p<0.05).
In spite of the limited duration of the follow up DMSA scan and relatively small size of the sample, follow up DMSA renal cortical scan is thought to be a useful method to evaluate the improvement of renal scars in acute urinary tract infection with VUR after surgical correction of the primary vesicoureteral reflux in infant before 2 years of age. Furthermore, study with larger sample size and long-term follow up examination of 12 months or more after anti-reflux surgery is recommended. And the study of DMSA scan comparison with CT and ultrasonography is recommended for further evaluation.
