인공 슬관절 감염의 발병율과 미생물학적 특성에 대한 고찰

Abstract
인공 슬관절 감염은 인공 슬관절 치환술 이후 가장 중요한 합병증이다. 최근 인공 슬관절 치환술의 빈도가 증가함에 따라 이에 대한 합병증의 사례도 증가하고 있다. 특히 감염의 경우 중증도가 높고, 경제적 부담도 커서 중요한 문제로 부각되고 있음에도 불구하고, 국내 연구 자료는 부족한 실정이다. 따라서 본 논문에서는 일개 대학병원의 인공 슬관절 감염에 대한 발병율 및 미생물 특성에 대하여 연구해 보았다.
본 연구는 2003년 1월 1일 부터 2010년 2월 5일까지 7년동안 대구 계명대학교 동산의료원에서 퇴행성 관절염으로 인공 슬관절 치환술을 받은 환자 를 대상으로 의무 기록을 중심으로 하여 후향적으로 조사하였다.
7년간 인공 슬관절 치환술을 받은 환자 386명중에서 일반적으로 사용되고 있는 진단 기준에 합당한 인공 슬관절 감염 환자는 6명으로 발생율은 1.6%였다. 일반적으로 인공 관절 감염 이라고 할 때, 심부 감염만 포함시키는 경우가 많으나 본 연구에서는 앞선 대규모 연구에서 흔히 사용되어온 진단 기준에는 포함되어 있지 않던 연부 조직 감염까지 포함시킴으로써 그 범위를 넓혀 조사하였다. 그 결과 인공 슬관절 감염에 합당한 환자는 30명으로 늘어났고, 발병율은 8%으로 증가 소견을 보였다. 이들 환자의 인구학적인 특징은 평균 나이 67.83세였고, 성별로는 여자가 21명(70%)이었으며, 평균 나이는 62.66세(표준편차 5.85) 였다. 인공 슬관절 감염 30예 중에서 25예에서 균배양을 시행하였고, 17예(68%)에서 균이 동정되었다. 동정된 균주는 Coagulase negative Staphylococcus (CNS)로 7예(28%)였고, Staphylococcus aureus (S.aureus) 4예(16%), Enterococcus fecalis 4예(16%), Acinetobacter baumanii 1예(4%), Pichiaanomola 1예(4%)였다. CNS중에서 4예(57.1%)에서 Methicilin에 내성이 있는 MRCNS였고, S.aureus의 모든 예에서 MRSA임을 확인하였다. 증세로는 통증이 23예(76%)로 가장 흔하였고, 종창 18예(60%), 발적 13예(43%) 순서로 흔하였다. 감염된 관절 제거 수술을 받고, 인공 슬관절 치환술을 받은 환자는 10예(33%), 죽은 조직 제거 수술만 받고, 인공 관절은 보존한 환자는 1예(3.3%)였으며, 항생제 단독 치료만 받은 환자는 19예(63%)였다. 가장 많이 투여한 항생제는 2세대 cephalosporins (83%)이었고, 연구 기간중 사망은 1예(3.3%)에서 발생하였다.
본 연구에서 인공 슬관절 감염 발생율은 1.6% 였고, staphylococci가 가장 흔한 원인 균주였다. Methicillin 내성 균주가 감수성 균주보다 더 많았으며, 사망률은 3.3%였다. 본 연구는 후향적으로 진행되었고, 적은 규모의 단일 연구 기관의 연구로 제한점이 있으므로 다기관 대규모 연구가 필요할 것으로 사료된다.
Prosthetic knee joint infection (PJI) is one of the most challenging complications of total knee replacement. Recently, the frequency of prosthetic knee joint infection is increasing as the number of primary arthroplasties. This increasing trend will continue dramatically in the near future, but there were relatively few recent descriptions of the incidence and microbial features of prosthetic knee joint infection in Korea. We therefore reviewed the incidence and microbial features of prosthetic knee joint infection in a single center in Korea.
We reviewed retrospectively the clinical notes and microbial records of patients with prosthetic knee joint infection who had admitted in Keimyung University Dongsan Medical Center from January 2003 to Feburary 2010.
During a seven-year peroid, prosthetic knee joint infections occurred in 6 (1.6%) of 368 patients who received a total knee arthroplasty at Keimyung University Dongsan Medical Center. In this study, we made more broad spectrum inclusion criteria for PJI. Generally, superficial infection was not included in PJI, but in this study, superficial infection was also regarded as PJI. With broad spectrum criteria for PJI, total thirty patients were PJI, incidence that involve superficial infection was 8%. The median age was 67.83 years. Twenty one patients(70%) were female and the mean age was 62.66 years (standard deviation (SD) 5.85). Seventeen (68%) of the twenty five patients were culture positive. Coagulase negative Staphylococcus is the most common pathogen (28%), followed by Staphylococcus aureus (16%), Enterococcus fecalis (16%), Acinetobacter baumanii (4%), Pichia anomola (4%). Four (57.1%) of the seven isolates of coagulase negative Staphylococcus were MRCNS and all of the isolates of S. aureus were MRSA. The most common symptom of prosthetic knee joint infection was pain (76%), followed by swelling (60%), redness (43%) and heatness (43%). Of the thirty episodes of PJI, 10 (33.3%) were treated with 2 staged exchange arthroplasty, 1(3.3%) with debridement, and 19(63.3%) with antibiotics alone in prosthetic knee infection. The most frequently used antibiotics was 2nd generation Cephalosporins (83%). Only one patient died during the study period and was related to the infection.
In our study, we found that incidence of prosthetic knee joint infection was 1.6%. In our report staphylococci represented the most frequent cause of PJI. Methicillin-resistant strains were more frequently isolated. Mortaility of our study was 3.3%. This study is retrospective and composed of small number, so further multicenter collaborations and large group trials are needed.
