저자들은 투명세포 신세포암에서 claudin-1, 7의 발현 유무와 임상적 지표에 대한 관련성을 조사하였다. 2000년 1월부터 2007년 12월까지 투명세포 신세포암으로 확진된 119명을 대상으로 하여 신세포암 조직을 claudin-1, 7로 면역조직화학염색을 하였다. 각각의 claudin의 발현 여부를 성별, 나이, 체질량지수, 종양크기, 술 전 TNM 병기, Furhman nuclear grade, 술 후 원격전이, 종양 특이 생존률과 연관성이 있는지 분석하였다. 전체 119명 중 claudin-1은 18명(15.1%), claudin-7은 31명(26.1%)에서 발현되었다. Claudin-1은 나이가 많을수록(p = 0.006), 종양크기가 클수록(p = 0.001), 술 전 T 병기가 높을수록(p = 0.044), 술 전 원격전이가 있을수록(p = 0.035), Furhman nuclear grade가 높을수록(p = 0.004) 발현되었다. Claudin-7은 Furhman nuclear grade가 높은 경우에만 발현되었다(p = 0.031). Claudin-1의 발현은 술 후 원격전이의 위험도와 연관성이 있었으나(p < 0.001), claudin-7에서는 연관성이 없었다(p = 0.445). 투명세포 신세포암에서 claudin-1의 발현은 임상적으로 술 후 원격전이에 대한 의미있는 예후인자로써의 가치가 있을 것으로 생각된다.
The authors of the present study investigated the correlations between the expression of claudin-1 and 7 in clear cell renal cell carcinoma (clear cell RCC) and clinical parameters. The subjects of this study were 119 patients who were confirmed for clear cell RCC between January 2000 and December 2007. Their RCC tissues were immunohistochemically stained with claudin-1 and 7. The correlations between the expression of claudin and such parameters as sex, age, body-mass index (BMI), tumor size, preoperative TNM stage, Furhman nuclear grade, postoperative distant metastasis, and cancer specific survival were analyzed. Claudin-1 was expressed in 18 (15.1%) and claudin-7 in 31 (26.1%). Claudin-1 was expressed in patients who were older (p=0.006), who had a greater tumor size (p=0.001), who had a higher preoperative T stage (p=0.044), who had preoperative distant metastasis (p=0.035), and who had a higher Furhman nuclear grade (p=0.004). Claudin-7 was expressed only in patients who had a higher Furhman nuclear grade (p=0.031). The risk of postoperative distant metastasis was associated with the expression of claudin-1 (p<0.001), but not to the expression of claudin-7 (p=0.445). In clear cell RCC, the expression of claudin-1 would have significant prognostic value for postoperative distant metastasis.
