항정신병약물로 치료받은 정신분열병 환자에서 세로토닌 수용체 유전자 다형성이 체중 증가와 치료 반응과의 관계에 미치는 효과

Abstract
체중 증가는 항정신병 약물을 장기적으로 투여 받는 환자들에서 나타나는 주요한 부작용 중의 하나이다. 임상에서 항정신병 약물을 투여 받은 환자들의 경우에 증상 호전이 일어나면서 체중 증가가 동반되는 경우를 흔히 볼 수 있는데, 이러한 사실은 항정신병 약물의 치료 반응과 체중 증가가 관련이 있을 것이라는 것을 시사한다. 본 연구의 목적은 항정신병 약물로 치료 받은 환자에서 체중 증가와 치료 반응이 서로 관련 있는지를 조사하고, 만약 이들 서로 간에 관련성이 있다면 이러한 관련성을 매개하는 유전자 요인이 있는지를 밝히고자 하는 것이다. 이를 위해서 항정신병 약물에 대한 치료 반응과 체중 증가에 모두 관여될 것으로 추정되는 세로토닌 수용체 유전자의 다형성을 조사하였다.
본 연구는 임상적 실제에서 일어나는 자연적인 연구이며 대상 환자들의 입원 당시 병록지를 검토하여 후향적으로 이루어졌다. 대상 환자는 일개 전신과 전문병원에서 입원치료 받는 환자들 중에 연령이 18―60세에 해당되고 DSM-IV의 정신분열병 진단기준을 만족시키며 본 연구에 대해 서면동의를 한 환자 164명이었다. 대상 환자들을 입원할 당시와 비교해서 퇴원할 당시에 체중 증가가 5% 이상인 경우는 ‘체중 증가군’, 5% 미만인 경우는 ‘체중 비증가군’으로 구분하였다. 치료 반응의 정도에 따라서 증상 호전이 없거나 경도의 호전만 있은 경우는 ‘비반응군’, 중등도 혹은 현저한 증상 호전이 있은 경우는 ‘반응군’으로 분류하였다. 본 연구는 항정신병 약물로 치료 받은 환자들에서 체중 증가군과 비증가군 사이에 치료 반응의 차이가 있는지를 확인하였고, 이들 양군 사이에 5-HT2A 수용체 유전자의 102 C/T 다형성, 5-HT2A 수용체 유전자 촉진자 부위의 -1438 A/G 다형성, 5-HT6 수용체 유전자의 267 C/T 다형성의 차이가 있는지를 조사하였다. 또한, 체중 증가와 치료 반응 사이의 관련성이 5-HT2A 수용체 유전자, 5-HT2A 수용체 유전자 촉진자 부위, 그리고 5-HT6 수용체 유전자의 다형성에 따라서 차이가 있는지를 조사하였다.
전체 164명의 환자 중 59명(36.0%)이 체중 증가군으로 분류되었다. 첫 발병 당시의 나이는 체중 증가군이 체중 비증가군보다 유의하게 더 많았다(p= 0.021). 과거 입원 횟수는 체중 증가군이 체중 비증가군보다 유의하게 더 적었다 (p= 0.012). 입원 당시의 체질량지수는 체중 증가군이 체중 비증가군보다 유의하게 낮았다 (p= 0.000). 입원 당시의 나이, 성별, 교육 수준, 결혼 상태, 가족력 유무, 입원 기간, 그리고 정신분열병의 아형 등에서는 양군 사이에 유의한 차이가 없었다.
체중 증가군에서 비반응군 보다 반응군이 유의하게 더 많았다(p< 0.05). 체중 증가군은 5-HT2A 수용체 유전자 촉진자 부위의 -1438 A/G 다형성의 A/A 유전자형과 연관이 있었다(p< 0.05). 체중 증가군과 체중 비증가군 사이에 5-HT2A 수용체 유전자의 102 C/T 다형성과 5-HT6 수용체 유전자의 267 C/T 다형성의 유전자형 분포에는 유의한 차이가 없었다. 체중 증가와 치료 반응 사이의 상관관계는 5-HT2A 수용체 유전자 촉진자 부위의 -1438 A/G 다형성의 A/A 유전자형을 가지고 있는 환자들과 5-HT6 수용체 유전자 267 C/T 다형성에서 C/C 유전자형을 가지고 있는 환자들에서만 관찰되었다.
결론적으로, 본 연구의 결과는 항정신병 약물로 치료 받은 환자에서 체중 증가가 치료 반응과 관련이 있으며, 이러한 관련성은 5-HT2A 수용체 유전자 촉진자 부위와 5-HT6 수용체 유전자의 특정 유전자 다형성과 관련이 있음을 시사한다. 향후에는 본 연구의 결과를 재현하기 위한 통제된 전향적 연구가 필요할 것으로 사료된다.
Weight gain is an important side effect of antipsychotic treatment. Some studies reported a positive association between symptom improvement and gain in body weight during antipsychotic treatment. This study investigated the effect of serotonin receptor gene polymorphism on the relationship between weight gain and therapeutic response in antipsychotic-treated schizophrenic patients.
This study was a retrospective chart review of 164 consecutively hospitalized patients with the diagnosis of schizophrenia (DSM-IV) treated with various antipsychotics (94% atypical antipsychotics) at a mental hospital. The patients were divided into two groups, weight gainers (weight gain ≥ 5%) and non-gainers (weight
gain < 5%) by percentile change of body weight at discharge compared to body weight at admission. The patients were also divided into another two groups, responders and non-responders, by responsiveness to antipsychotics according to a four-point scale used in previous studies : responders included moderate to marked responded patients and non-responders included none to minimal responded patients. This study investigated the differences of 102C/T polymorphism of 5-HT2A receptor gene, -1438A/G polymorphism of 5-HT2A receptor gene promoter, and 267C/T polymorphism of 5-HT6 receptor gene between weight gainers and non-gainers. This study also investigated the relationship between weight gain and therapeutic response according to polymorphisms of 5-HT2A receptor gene, 5-HT2A receptor gene promoter, and 5-HT6 receptor gene.
Among the total patients of 164, 59 (36.0%) were categorized as weight gainers. The age at first onset in weight gainers was significantly older than in non-gainers (p= 0.021). The number of previous hospitalization in weight gainers was significantly lower than in non-gainers (p= 0.012). The baseline Body Mass Index (BMI) in weight gainers was significantly lower than in non-gainers (p= 0.000). There were no significant differences between the two groups in age at admission, sex, education, marital status, family history, duration of admission, and type of schizophrenia. Weight gainers were significantly more frequent in responders than non-responders (p< 0.05). Weight gainers were associated with A/A genotype of -1438G/A polymorphism of 5-HT2A receptor gene promoter (p< 0.05). There were no significant differences in genotype frequencies of 102T/C polymorphism of 5-HT2A receptor gene and 267T/C polymorphism of 5-HT6 receptor gene between weight gainers and non-gainers. The relationship between weight gain and therapeutic response was found only in A/A genotype carriers of -1438A/G polymorphism of 5-HT2A receptor gene promoter and C/C genotype carriers of 267C/T polymorphism of 5-HT6 receptor gene.
These results suggest that weight gain is associated with therapeutic response in antipsychotic-treated schizophrenic patients and the polymorphism of 5-HT2A receptor gene promotor and 5-HT6 receptor gene may effect on this association. Further research is needed to replicate these results.
