자궁경부암 선별검사로서 인유두종바이러스 DNA 검사의 임상적 유용성

Abstract

자궁경부암은 긴 전암단계를 통해 조기진단이 가능하며 선별검사로서 세포진 검사를 통해 발생을 줄이려는 노력에도 불구하고 여성 암 중 5.7%로 6위를 차지하며 매년 3000명 이상의 새로운 환자가 발생하고 있다. 자궁경부암 선별검사로서 세포진 검사의 낮은 민감도를 보완하기 위해 인유두종바이러스 DNA 검사가 각광을 받고 있는 중에 저자는 Hybrid capture II 검사(Digene, USA)와 세포진 검사를 같이 시행한 환자들을 대상으로 인유두종바이러스 DNA 검사의 유용성을 알아보고자 하였다. 총 195명을 대상으로 연구를 진행하였으며 그 중 조직학적 진단이 있는 51명의 환자 중 CIN II 이상의 고등급 병변을 가진 환자가 32명(62.7%)으로 다수를 차지하였다. 32명 모두에서 인유두종바이러스 DNA 검사가 양성(100%)으로 확인되었고 세포진 검사도 비교적 높은 민감도(96.9%)를 보였으나 SCC에서 위음성을 보인 1예가 있었다. CIN II 이상의 고등급 병변을 보인 32명 중 22%에서 세포진 검사가 ASCUS로 나타나 조직 검사와 세포진 검사는 다소 차이가 있었다. 인유두종바이러스 DNA 검사에 양성이지만 조직학적 진단이 이루어지지 않은 62명에 대해 1년간 추적 관찰한 결과, 그 중 7명에서 세포진검사 상 진행되는 소견을 보였고, HSIL로 나타난 2명에서 조직 검사한 결과 1 예는 CIN II로 확인되었고 1예는 양성(benign) 병변인 것으로 확인되었다. 그리고 두 검사 모두 음성이었던 1예에서는 1년 후 세포진검사에서 LSIL로 진행되었고 동시에 실시한 인유두종바이러스 DNA 검사 결과 양성, 그리고 조직검사상 CIN II로 확진되었다. 인유두종바이러스 DNA 검사는 높은 민감도와 음성예측도를 보이므로 자궁경부암 선별검사로서 유용할 것으로 생각되며 낮은 음성예측도로 인한 문제를 위해 세포진 검사와의 적절한 병용이 필요하겠으며 이를 위해 다양한 환자군에 대한 연구를 통해 임상 지침을 확립해야 할 것으로 생각된다.

cervical cancer is 6th prevalent invasive cancer in Korean women and more than 3,000 new patients develop per year, although the enforcement of the screening program by cervical cytology (Pap smear), False negativity of conventional cytology is problematic and the sensitivity of cytology was highly variable. Therefore, newly developed tests based on molecular method to detect Human paillomavirus (HPV) DNA have been introduced. Author evaluated the sensitivity with specificity and efficacy of HPV DNA test as a screening test for cervical cancer according to the biopsy which is a gold standard method. Thirty？]two of 51 (62.7%) patients who performed cervical biopsy showed higher grade of severity than CIN II in biopsy and were all positive (100%) in HPV DNA test. Cytology showed sensitivity of 96.9% but a case of false？]negative was found in SCC patient and seven of 32 (22%) patient who showed high grade cervical lesion for cytology were confirmed as ASCUS. Sixty？]two patients with HPV DNA positive but with no results of cervical biopsy had a year of prospective follow？]up, seven revealed advancement of cytological findings for 1 year. One of two patients with HSIL confirmed to have CIN II and the other as inflammatory lesion in biopsy after a year. Since HPV DNA test showed higher sensitivity and negative predictive value for the detection of high？]grade cervical lesions than cytology, it would be clinically efficient used as a primary screening test for cervical cancer. In addition, more effective guidelines for combined uses with cytology would need to be established and further studies would also be required.
