(The) risk factors and outcome of acute kidney injury in the intensive care units evaluated by the RIFLE classification

Abstract
Background: Acute kidney injury (AKI) is a common and serious complication in critically ill patients, especially, in the intensive care unit (ICU). Recently the Acute Dialysis Quality Initiative group published the RIFLE classification, a new consensus and evidence-based definition for AKI. The aims of this study were to evaluate the occurrence rate of AKI using RIFLE classification, to define the associated factors of AKI and hospital mortality.
Method: We constructed a retrospective study of all ICU patients during a 6 month period(July 2007-Dec 2007) at the Keimyung University Dongsan Hospital. We excluded the patients who stayed in the ICU for less than 48 hours, patients readmitted in the ICUs, admitted after uncomplicated operation and had a history of chronic renal failure requiring renal replacement therapy. AKI was classified according to the RIFLE (Risk, Injury, Failure, Loss, End-stage kidney disease) and illness severity was evaluated by SOFA during the first 7 days of ICU admission.
Results: Of the 378 patients, AKI occurred in 156 patients (41.3%) during their ICU stay, with maximum RIFLE class Risk, class Injury, and class Failure in 13.8%, 12.4%, 15.1%, respectively. The age, sex, reason for admission and length of hospital stay were not significantly different among the patients with AKI and non-AKI. In a univariate analysis, the proportion of medical admission and SOFAmax score were significantly higher in patient with AKI group than non-AKI group. But, these significances were disappeared in a multivariate analysis. The overall mortality of ICU patients was 25.7% (97 out of 378 patients). Hospital mortality was 19% for non-AKI patients, 29% for Risk patients, 36% for Injury patients and 51% for Failure patients. In a multivariate analysis, mean age, occurrence of AKI, SOFAmax score, pulmonary disease and malignancy were independent risk factors for hospital mortality.
Conclusions: In this ICU patients, acute kidney Risk, Injury, Failure as defined by newly developed RIFLE classification is associated with increased hospital mortality. The RIFLE classification appears to be a simple and useful clinical tool to detect and stratify the severity of AKI, and possibly predict outcome.
배경: 급성 신손상은 특히, 중환자실 입원 환자에 있어 흔하면서도 중대한 합병증이다. 최근 Acute dialysis quality Initiative (ADQI) group에서는 급성 신손상에 대해 새로운 주제와 객관적 자료에 입각한 정의로 RIFLE분류법을 제시하였다. 본 연구에서는 중환자실 환자를 대상으로 RIFLE분류법을 적용한 급성 신손상의 발생빈도, 급성 신손상 발생과 위험인자 및 예후 관련인자를 평가하고자 하였다.
방법: 계명대학교 동산병원에서 2007년 7월부터 2007년 12월까지 6개월 동안 중환자실에 입원한 환자를 대상으로 후향적으로 분석하였다. 만성신부전으로 투석 받는 환자, 중환자실 입원 기간이 48시간 이내인 환자, 계획된 수술 후 중환자실에 관행적으로 입원한 환자는 대상에서 제외되었다. 급성 신손상은 RIFLE분류법에 따라 분류하였고, 질환의 중증도는 중환자실에 입원하는 동안 7일간의 SOFA점수체계를 계산하여 평가하였다.
결과: 총 378명의 환자가 연구에 포함되었으며 이들의 평균 나이는 62.6±15.9세, 남자비율이 62.6%, 내과적 입원이 82.5%, 평균 SOFA score는 17.2±17.2이었다. 이 중 156명(41.3%)이 중환자실에서 급성 신손상이 발생하였다. RIFLE 분류법에 따라 위험군은 13.8%, 손상군은 12.4%, 신부전군은 15.1%이었으며 58.7%는 급성 신손상이 발생하지 않았다. 급성 신손상이 동반된 환자와 동반되지 않은 환자 간에 나이, 성별, 입원기간, 입원 원인에는 유의한 차이가 없었으나 급성 신손상이 있는 환자에서 내과적 입원 환자 비율 및 최대 SOFA 점수가 유의하게 높았다. 그러나 이러한 차이는 다변량 분석에서는 유의한 차이를 보이지 않았다. 전체 환자의 사망률은 25.7%였으며, 급성 신손상이 없는 환자의 사망률은 19%인데 비해 급성 신손상 위험군 29%, 손상군 36%, 신부전군 51%로 신손상의 정도가 심할수록 입원 사망률이 유의하게 높았다. 입원 사망률과 관련된 위험인자를 평가하는 다변량 분석에서는 환자 연령, 급성 신손상의 발생, 최대 SOFA 점수, 호흡기 질환, 악성 종양이 독립적인 위험 인자였다.
결론: 중환자실 환자에 있어 RIFLE분류법으로 정의된 급성 신손상의 위험군, 손상군, 신부전군의 급성 신손상의 중증도가 심할수록 입원 사망률이 유의하게 증가 하였으며 급성 신손상의 발생은 환자 사망의 독립적인 위험 인자였다. 따라서, RIFLE분류법은 급성 신손상 환자에서 명확하고 단일화된 정의를 통해 신손상을 평가하고 환자의 중증도와 예후를 측정하는데 간단명료하면서도 매우 유용한 평가 도구로 생각된다.
