한국판 우울증 전귀 척도(Clinically Useful Depression Outcome Scale; CUDOS)의 표준화 연구 : 신뢰도와 타당도 검정

Abstract

본 연구의 목적은 지금까지 우울증의 호전 정도를 평가하기 위해 Hamilton Depression Rating Scale (HDRS) 등을 이용해 왔지만, 시간이 많이 소요되고 힘이 많이 들어 평가자나 환자에게 부담을 주었다. 본 연구는 Zimmerman 등이 최근 개발하였고, 임상 진료에서 간편하면서도 손쉽게 사용할 수 있는 우울증 전귀 척도(Clinically Useful Depression Outcome Scale, CUDOS)를 표준화하고자 한다. 본원 정신과에서 외래 치료 혹은 입원 치료를 받았던 환자들과 대구시 소재 개인병원에서 입원치료를 받았고, DSM-IV에 의해 주요우울증으로 진단된 환자 70명을 대상으로 CUDOS와 CGI-S, PHQ-9, CES-D, HDRS를 시행하여 신뢰도 및 타당도를 측정하였다. CUDOS의 신뢰도 검정 결과 Cronbach’s alpha 계수는 0.911으로 높게 나타났고, 반분 신뢰도 및 검사-재검사 신뢰도도 높았다. 문항-총점 간의 상관관계는 1항목을 제외한 대부분의 항목에서 높은 상관관계가 있었다. 타당도에서는 우울증 환자군의 CUDOS 평균 점수가 대조군보다 유의하게 높았다. CUDOS와 비교측정도구인 CGI, PHQ-9, CES-D, HDRS 각각의 상관계수가 매우 높았고, 요인분석에서는 5개의 요인으로 나뉘었고, 설명량은 총 71.6%이었다. 결론적으로, CUDOS는 신뢰도와 타당도에서 높은 수준을 보였고 임상에서 우울증 환자에게 손쉽고 간편하게 사용할 수 있는 유용한 척도로 기대된다.

Objectives : The Clinically Useful Depression Outcome Scale (CUDOS) was developed to be brief, clinically useful, reliable, and valid self-report questionnaire for the evaluation of depression outcome. This study planned to standardize Korean translation of the CUDOS. Methods : The subjects were 70 patients, who were hospitalized or visited outpatient clinic for major depressive disorder diagnosed by DSM-IV criteria, and 50 normal controls. The depressive subjects were also administered HDRS, CES-D, PHQ-9, and CGI and normal controls were administered the same battery. Results : The Cronbach’s alpha coefficient of the CUDOS were high, Cronbach's α = .911. The CUDOS had high internal consistency and test-retest reliability. The correlation coefficients between the CUDOS and CGI-S, HDRS, CES-D, and PHQ-9 were respectively high. Especially, the CUDOS was also highly correlated with interviewer ratings of the severity of depression. To identify factor structure of the CUDOS, factor analysis was performed. There were five factors above eigen value 1.0, which explained 71.635% of variances. Conclusion : These results suggests that the CUDOS is reliable and valid self-report questionnaire that is feasible to incorporate into routine clinical practice for the evaluation of depressive symptoms.
