Association between Tumor Necrosis Factor-α Polymorphisms and Coronary Artery Disease

Abstract
Coronary artery disease (CAD) caused by atherosclerosis is a chronic inflammatory disease. It has been well established that proinflammatory cytokines play a critical role in the development of atherosclerosis. Tumor necrosis factor-α (TNF-α) is expressed in atherosclerotic plaque and plays an essential role in the development and progression of atherosclerosis. In this study, we examined whether polymorphisms of TNF-α gene promoter may affect the incidence of CAD in Korean population.
One hundred ninety seven patients, who previously underwent coronary artery stent and 404 control subjects were participated in this study. We determined the genotype for the G/A polymorphisms at position -238, the C/T polymorphisms at position -857 and the C/A polymorphisms at position -863 in TNF-α gene by polymerase chain reaction and sequencing analysis. The frequencies of the G/G, G/A and A/A genotype at position -238 were 85.8%, 14.2% and 0% in CAD group and 91.8%, 7.9% and 0.3% in control, respectively. There was a significant association between G/A polymorphisms at position -238 in TNF-α gene and CAD (dominant; OR=1.87 95% CI=1.10-3.20 P=0.02 and co-dominant; OR=1.93 95% CI=1.13-3.32 P=0.04). The A allele carriers had significantly increased CAD risk compared to the G allele (OR=1.74; 95% CI=1.04-2.92 P=0.03). However, polymorphisms at position -857 and -863 in TNF-α gene were not associated with CAD. According to haplotype-based analysis, the frequencies of haplotype ACC at position -238, -857 and -863 in the TNF-α gene differed significantly between CAD and control group (dominant; OR=1.86 95% CI=1.09-3.18 P=0.02 and co-dominant; OR=1.77 95% CI=1.05-2.98 P=0.03).
The present study showed that the -238/G→A single nucleotide polymorphism of TNF-α contributed to the development of CAD. Our data suggest the possibility that carrying an -238A allele is significantly associated with the presence of CAD and this polymorphism of TNF-α could be used as predictor for CAD in Korean. 관상동맥질환은 죽상동맥경화증에 의해 일어나는 만성 염증 질환이다. 죽상동맥경화증의 발병에 염증성 물질들이 중요한 역할을 하는 것은 잘 알려져 있다. 종양괴사인자-알파(TNF-α)는 죽상판에서 산생되는 전염증성 사이토카인으로 혈관 염증을 가속화하여 관상 동맥 협착을 유도하여 결론적으로 관상동맥질환에 기여하게 된다.
TNF-α 유전자 다형성이 허혈성 심질환의 예측인자로서의 유용성을 알아보기 위해 관상동맥질환으로 인해 스탠트 삽입술을 시행 받았던 경력이 있는 197명의 환자들과 404명의 대조군을 대상으로 TNF-α 촉발부위 -238 위치와 -857 위치 및 -863 위치의 유전자 다형성을 검사하였다. 환자군에서 TNF-α 촉발부위 -238 위치의 유전자 다형성인 경우 G/A 유전형이 대조군과 비교하여 의미 있게 증가하였으며(dominant; OR=1.87 95% CI=1.10-3.20 P=0.02 and co-dominant; OR=1.93 95% CI=1.13-3.32 P=0.04), 또한 allele 분석에서도 A allele이 G allele에 비해 관상동맥질환의 위험도 증가와 의미있는 연관성을 보였다(OR=1.74; 95% CI=1.04-2.92 P=0.03). 그러나 TNF-α 촉발부위 -857 위치와 -863 위치에서의 유전자 다형성은 관상동맥질환과 의미 있는 관련성을 보이지 않았다.
본 연구결과 TNF-α 촉발부위 -238 위치의 G/A 유전자 다형성이 허혈성 심질환의 발병과 연관성이 있을 것으로 예상된다. 따라서 TNF-α의 이러한 유전자 다형성이 허혈성 심질환의 유용한 예측인자가 될 수 있으리라 사료되며 이를 규명하기 위한 대규모 연구가 필요할 것으로 간주된다.
