렙틴 저항성을 가진 비만 쥐에서 뇌실로 주입한 MTⅡ와 Naloxone의 효과

Abstract
생명체는 체지방의 항상성을 유지하기 위하여 지방세포 에너지 과잉축적의 신호로 렙틴이라는 물질을 분비하게 되고 렙틴은 시상하부에 작용하여 식욕을 감소시키고 에너지 소비를 증가시켜서 체지방을 감소시키는 음성 되먹이기 기전이 작동된다. 그러나 마른 사람이나 실험동물에 렙틴을 주사하면 현저한 체지방 감소효과가 나타나지만 비만한 사람이나 실험동물에서는 이미 혈중 렙틴 농도가 높아져 있으며 렙틴을 주사하여도 식욕억제효과가 나타나지 않는 렙틴저항성이 나타나 있다. 렙틴 저항성의 일차적인 원인은 시상하부의 궁상핵에서 렙틴 신호전달과정의 결함으로 생각하며, 궁상핵을 우회하여 렙틴 효과를 만들어내고자 하는 연구들이 진행되어 왔다. 제 3/4형 melanocortin 수용체에 작용하는 MTⅡ를 뇌실로 주사하면 렙틴유사 효과가 시간이 지나면서 감소하는 tachyphylaxis 현상이 나타나기 때문에 tachyphylaxis를 방지하면 melanocortin system을 목표로 하는 비만치료가 효과적으로 사용될 수 있을 것이다.
본 연구에서는 렙틴 저항성을 나타내는 비만 모델에서 뇌실로 주입한 MTⅡ의 렙틴유사 효과를 살펴보고, MTⅡ 투여에 따르는 tachyphylaxis 현상을 MTⅡ의 melanocortin system을 통한 자극 지속에 따른 보상기전으로 인한 agouti-related peptide (AgRP)의 증가라는 가설을 세우고, AgRP의 비특이적인 antagonist인 naloxone을 함께 투여하였을때 MTⅡ의 식욕억제 효과를 지속적으로 억제할 수 있는지를 관찰하고자 하였다.
실험동물로는 대조군으로 Spargue-Dawley 쥐를 사용하였으며, 비만모델로 4개월간의 고지방식이 비만쥐와 유전적 요인으로 비만이 유발된 Otsuka Long-Evans Tokushima Fatty (OLETF) 쥐를 사용하였다. MTⅡ와 naloxone을 뇌실로 삽입된 brain infusion kit와 연결된 osmotic minipump를 사용하여 1주일간 투여하면서 매일 식이섭취와 체중변화를 측정하였다.
고지방식이로 유발된 비만쥐와 OLETF 쥐 모두에서 정상 대조군에 비하여 뇌실로 주입한 렙틴에 의한 식이섭취 억제가 감소한 렙틴저항성을 확인할 수 있었다. 고지방식이 비만쥐에 MTⅡ를 주입한 결과 초기에는 식욕억제가 나타났으나 시간이 지남에 따라 그 효과가 감소하는 tachyphylaxis 현상이 나타났다. OLETF 쥐에 MTⅡ를 주입한 결과도 고지방식이 쥐와 유사한 결과를 나타내었다. 그러나 OLETF 쥐에서 MTⅡ와 naloxone을 함께 투여한 경우에는 MTⅡ의 식욕억제, 체중감소 및 복강내 지방 감소효과가 증가되었다. 정상 대조쥐에서는 뇌실로 주입한 naloxone의 식욕억제 효과는 나타나지 않았으며 MTⅡ와 함께 투여한 경우에도 MTⅡ의 효과와 차이가 없는 결과를 나타내었다. OLETF 쥐에서 real-time PCR로 분석한 시상하부 proopiomelanocortin (POMC) 발현은 naloxone 처치군에서 유의하게 증가하였다. AgRP의 발현은 MTⅡ, naloxone, 및 혼합 투여군에서 증가하는 경향을 나타내었다. Neuropeptide Y (NPY)의 발현은 AgRP와 유사한 경향을 나타내었다. 제3/4형 melanocortin 수용체 발현은 정상쥐에서 MTⅡ처치에 의하여 감소하는 경향이었으나 OLETF 쥐에서는 차이가 없었다.
요약하면, 렙틴저항성이 발생한 고지방식이 비만쥐와 OLETF 비만쥐에서 뇌실로 1주일간 주입한 MTⅡ의 렙틴 유사효과는 정상 대조쥐에 비하여 증가하지 못하였으며 tachyphylaxis를 나타내었다. Naloxone은 대조군과 OLETF에서 단독으로는 식욕억제 효과가 나타나지 않았으나 OLETF 쥐에서 MTⅡ와 함께 투여하였을때 MTⅡ의 식욕억제, 체중감소 및 복강내지방감소효과가 증가되었다. 이 결과는 melanocortin system을 목표로하는 비만 약물을 사용할 때 naloxone을 함께 사용하는 것이 효과적임을 시사하는 것이다. Eventhough leptin, an adipocyte-derived hormone, has a potent anti-obesity effect in lean subjects, leptin is not widely used in the clinical field because of leptin resistance. Thus, circumventing arcuate nucleus of the hypothalamus, a site responsible for leptin resistance, has been evaluated for treatment of obesity. However, chronic treatment of MTⅡ, a synthetic agonist of melanocortin 3/4 receptor, induces a tachyphylaxis. In the present study, we evaluated whether naloxone, a non-specific agouti-related peptide (AgRP) antagonist, increases the anorexic effect of MTⅡ in Otsuka Long-Evans Tokushima Fatty (OLETF) rats.
We have measured food intake following intracerebroventricular (i.c.v.) infusion of MTⅡ and/or naloxone in OLETF rats. Sprague-Dawley rats were used as a control group.
The anorexic effect of i.c.v. MTⅡ infusion was gradually blunted throughout days of treatment in OLETF rats, indicating the development of tachyphylaxis. In control rats, noloxone alone has shown a tendency of decrease in AgRP expression in the hypothalamus but failed to induce anorexia. Moreover, there was no additional anorexic effect when naloxone was treated together with MTⅡ. In OLETF rats, naloxone alone has not shown any anorexic effect despite of increased POMC expression in the hypothalamus. However, naloxone sensitized the anorexic effect of MTⅡ when treated together.
These results suggest that naloxone augmented the anorexic effect of MTⅡ when treated together in OLETF rats, while naloxone alone did not. These results suggest that combination therapy of naloxone and melanocortin receptor activator would be an effective modality for treatment of obesity.
