Prognostic significance of hypermethylation and expression of p16, p21, and p53 genes in diffuse large B-cell lymphoma

Abstract
Diffuse large B-cell lymphoma (DLBCL) is the largest category of aggressive lymphomas and accounts for approximately 30―40% of non-Hodgkin's lymphomas. Biologically and clinically, DLBCL shows heterogeneity and significant histologic and molecular prognostic markers are not well known. Hypermethylation of cytosine residues of promoter CpG islands represses gene transcription, and is an alternative mechanism of gene inactivation. p16 is frequently silenced by CpG island hypermethylation in solid tumors and hematologic malignancies. In DLBCL, CpG hypermethylation of the p16 promoters has been found in 27―46% of cases. Also, hypermethylation of p21 and p53 are reported in various tumors. But studies for correlation between hypermethylation and expression of p16, p21, and p53 gene and clinical indicators and survival are rare. In this study, hypermethylation of p16, p21 and p53 gene and protein expression of p16, p21, p53, p63, and Rb in DLBCL were investigated with 91 paraffin blocks. The correlations between the results of immunohistochemical stains and methylation- specific PCR and clinical features were analysed by χ² test, Spearman's correlation analysis, and patient's survival by Kaplan-Meier method. Hypermethylation of p16, p21, and p53 were found in 35.6%, 6.1%, and 3.5%, respectively. In immunohistochemical staining, p16 expression was not noted in 32.9%. In Spearman's correlation analysis, p16 expression was correlated with stage (r = -0.315, p = 0.019) and IPI risk group (r = -0.291, p = 0.034). Survival analysis showed a tendency of longer survival in group exhibiting hypermethylation of p16 gene. χ² test revealed that promotor hypermethylation of p21 gene was very strongly correlated with recurrence (p < 0.001). Survival analysis showed a strong tendency of longer survival in p21 positive group (p = 0.057). Hypermethylation of p53 gene was correlated with presence of B symptoms (r = 0.291, p = 0.029). Reduced Rb expression was noted in 15% of DLBCL. Rb expression was inversely correlated with IPI risk group (r = -0.302, p = 0.028).
Hypermethylation of p16 gene was more common than hypermethylation of p21 or p53 in DLBCL. Hypermethylation of p21 gene was highly related to recurrence and reduced expression of p21 protein showed a strong tendency of poor survival. For an individual patient with DLBCL, if the tumor shows hypermethylation of p21 gene, it may make to predict recurrence. If the tumor shows reduced p16, Rb expression, and/or methylation of p53 gene, it is important to consider that this markers are related to higher IPI, which help to predict outcome of patients. 광범위 대세포 B 림프종(diffuse large B cell lymphoma)은 공격적인 림프종 중 가장 많은 수를 차지하며, 비호지킨 림프종의 30―40%를 차지한다. 광범위 대세포 B 림프종은 다양한 임상 양상을 보이며 예후와 강하게 관련된 조직학적인 표지자가 잘 알려져 있지 않다. p16 촉진자의 시토신(cytosine)은 정상적인 조직에서는 과메틸화되어 있지 않다. 그러나 각종 고형성 종양과 혈액종양에서 CpG island의 과메틸화에 의한 p16의 기능 억제가 흔하며 광범위 대세포 B 림프종에서도 p16의 이상 과메틸화가 27―46%로 보고되어 있다. 또한, p21 및 p53의 과메틸화 역시 다양한 종양에서 보고되어 있다. 그러나 광범위 대세포 B 림프종에서 관찰되는 과메틸화와 임상 양상 및 예후와의 관계에 대한 연구는 드물다. 이 연구에서는 광범위 대세포 B 림프종 91례에서 p16, p21, p53의 CpG island의 과메틸화 정도와 p16, p53, p63, p21, Rb 단백의 발현 정도를 파라핀 포매조직을 이용하여 평가하였으며, 이들 지표 사이의 관계 및 임상 지표와의 관계에 대하여 비교 분석하였다. 실험 결과와 임상지표와의 관계를 알기 위하여 χ²검정과 스피어만의 순위 상관관계분석을 시행하였고 예후에 미치는 영향을 평가하기 위하여 Kaplan-Meier법으로 생존분석을 시행하였다. 연구 결과 35.6%에서 p16 유전자의 메틸화가 관찰되었으며, p21과 p53의 메틸화는 각각 6.1%와 3.5%로 관찰되었다. 면역조직화학염색 결과 p16의 단백 발현이 32.9%에서 관찰되지 않았고, p16에 대한 발현이 낮을수록 환자의 병기(r = -0.315, p = 0.019)와 국제예후지표(r = -0.291, p = 0.034)가 높았다. p21에 대한 면역조직화학염색 결과 64.8%에서 p21 단백이 발현되었다. p21의 CpG island의 과메틸화는 재발과 상관성이 있었고(p < 0.001), p21 단백 발현이 저하되어 있을 때 환자의 생존 기간이 더 짧은 경향을 나타내었다(p = 0.057). p53 유전자의 과메틸화는 B 증상 존재와 관련이 있었다(r = 0.291, p = 0.029). Rb 단백에 대한 면역조직화학염색 결과 15.4%에서 약하게 발현되었으며, Rb에 대한 발현이 낮을수록 국제예후지표가 높았다(r = -0.302, p = 0.028).
이 연구 결과 광범위 대세포 B 림프종에서 p16의 메틸화가 p21이나 p53의 메틸화보다 더 흔히 발생한 것을 알 수 있었다. p21 유전자의 과메틸화는 드물게 관찰되나 재발과 밀접하게 관련되어 있었으며, p21 단백의 발현이 저하되어 있을 때 예후가 더 나쁜 경향을 강하게 나타내었다. DLBCL 환자의 종양에서 p16 또는 Rb 단백의 발현 저하가 관찰되는 경우나 p53 유전자의 과메틸화가 관찰되는 경우에는 이러한 인자들이 높은 IPI와 관련되어 있어 예후에 나쁜 영향을 미칠 수 있다는 점을 고려하여야 할 것으로 생각한다.
