Liver-directed gene therapy of diabetic rats using an HVJ-E vector containing EBV plasmids expressing insulin and GLUT-2 transporter

Abstract
Diabetes mellitus has long been considered to be curable with insulin gene therapy. However, several unsolved problems currently hamper the use of insulin gene therapy in clinical practice. First, to regulate blood glucose levels without risking the development of hypoglycemia, any insulin secretion derived from the vector construct must be regulated by changes in blood glucose levels. Second, the gene expression vector used must be capable of prolonged expression. Finally, an efficient and minimally invasive vector delivery system is required. To overcome these issues, we injected the liver of streptozotocin-induced diabetic rats with hemagglutinating virus of Japan- envelope (HVJ-E) vectors containing Epstein-Barr virus (EBV) based-plasmids encoding the genes for insulin and the GLUT-2 transporter. Efficient delivery of the genes was achieved with the HVJ-E vector, and the use of the EBV replicon vector led to prolonged hepatic gene expression. Fasting blood glucose levels were normalized for at least 3 weeks as a result of the gene therapy. Cotransfection of GLUT-2 with insulin permitted the diabetic rats to regulate their blood glucose levels upon exogenous glucose loading in a physiologically appropriate manner and improved postprandial glucose levels.
Moreover, cotransfection with insulin and GLUT-2 genes led to in vitro glucose-stimulated insulin secretion that involved the closure of KATP channels. The present study represents a new way to efficiently deliver insulin gene in vivo that is regulated by ambient glucose level with prolonged gene expression. This may provide a basis to overcome limitations of insulin gene therapy in humans. 오랫동안 당뇨병을 근원적으로 치료하기 위한 노력으로 인슐린 유전자 치료가 활발히 연구 되고 있다. 그러나, 인슐린 유전자 치료법을 실제 임상에 적용하기 위해서 해결되어야 할 문제점은 첫째로, 전달된 인슐린유전자가 혈당에 반응하여 생리적으로 분비되어야하고, 둘째로, 전달된 유전자가 장기적으로 발현되며, 마지막으로, 유전자 전달법이 비침습적이어야 한다는 것이다. 이러한 문제점을 극복하기 위해서, 저자는 Hemagglutinating virus of Japan-envelope (HVJ-E) 벡터를 이용하여 인슐린 유전자와 글루코스 트랜스포터 2 (GLUT-2) 유전자를 함유한 Epstein-Barr virus plasmids (EBV)를 스트렙토조토신으로 당뇨병이 유발된 백서의 간에 주입하였다. 유전자 전달상의 효율은 HVJ-E 벡터를 사용하여 증가되었고, EBV plasmids를 사용함으로써 백서의 간으로 전달된 인슐린 유전자가 장기간 발현되었다. 인슐린 유전자 치료에 의해서 공복시 혈당은 3주 이상 정상으로 유지 되었다. 당부하검사상 인슐린과 GLUT-2 유전자를 함께 전달한 당뇨병 백서는 외부에서 포도당 주입에 따른 혈당조절 능력이 개선되었고, 식후 2시간 혈당도 인슐린 유전자 치료만 한 당뇨병 백서 보다 향상 되었다. 더욱이, 전기 생리학 검사와 포도당 및 글리벤클라마이드 자극 인슐린 분비능 검사는 인슐린과 GLUT-2 유전자를 함께 전달한 당뇨병 백서에서는 기능적인 KATP 통로가 생성되어 혈당에 반응하여 인슐린이 분비됨을 보여 주었다. 따라서, 본 연구는 HVJ-E 벡터와 EBV를 이용하여 인슐린 유전자와 GLUT-2 유전자를 당뇨병이 유발된 백서의 간에 주입할 경우 효과적으로 인슐린유전자와 GLUT-2 유전자를 효과적으로 전달 할 수 있고, 장시간 전달된 유전자가 발현하여 혈당을 조절 할 수 있음을 확인 하였다. 본 연구에 사용한 인슐린 유전자 치료법은 기존의 인슐린 유전자 치료법의 문제점을 극복할 수 있음을 보여 주었다.
