무모생쥐에서 사해염이 급성 피부장벽손상 후 장벽회복에 미치는 영향

Abstract
세라마이드는 각질세포사이의 공간을 채우는 각질층 지질의 주성분으로, 각질층 지질의 26% 정도를 차지한다. 각질층 지질은 피부장벽 기능과 수분을 함유하는 기능을 하며 그 중에서도 특히 세라마이드가 이 두 가지 기능에 주 결정인자로 작용하여 손상된 급성, 만성 피부질환 시에 장벽회복에 도움을 주는 것으로 이미 알려져 있다.
사해염(dead sea salt)은 많은 양의 염화마그네슘, 염화칼슘, 염화나트륨, 염화칼륨, 브롬 및 황산칼슘을 함유하고 있으며 정확한 기전은 알려져 있지 않으나 편평세포 증식 억제, 표피 세포의 유사 분열 억제, 피부의 상재균총 제거 및 각질 세포의 항원 발현을 억제하는 면역 작용 등으로 건선 및 아토피 피부염과 같은 피부과적 질환 치료에 그 유용성이 보고되었다. 이런 배경을 바탕으로 최근에 사해염을 함유한 보습제를 비롯해 많은 사해염 함유 제품들이 개발되어 여러 피부질환에 보조적 치료제로 사용되고 있다. 그러나, 보습제의 하나의 중요 구성성분으로 함유된 사해염이 손상된 피부 장벽회복에 대한 직접적인 영향은 아직 잘 알려진 바가 없다. 이 연구는 무모생쥐의 배부를 5 등분하고 각각에 테이프 스트리핑 단독 처치, 테이프 스트리핑 후 순수 크림, 테이프 스트리핑 후 사해염(1%) 함유 크림, 테이프 스트리핑 후 3 형 세라마이드(2%) 함유 크림, 그리고 테이프 스트리핑 후 사해염(1%)와 3 형 세라마이드(2%) 혼합 크림을 도포한 후 테이프 스트리핑 처치 직후, 2.5, 6, 10, 24, 48 시간 뒤 경표피수분손실을 각각 측정하여 장벽 회복률을 구하여 시간 경과에 따른 장벽회복 양상을 관찰하고, 세라마이드와 급성 피부장벽손상 후 장벽회복에 미치는 영향을 비교 분석하여 다음과 같은 결과를 얻었다.
첫째, 사해염과 3 형 세라마이드 혼합 크림을 도포하였을 때 각각을 단독 도포한 경우보다 장벽 회복률이 높았다. 따라서 사해염과 3 형 세라마이드 혼합 크림이 급성 혹은 만성적인 장벽손상이 나타나는 피부 질환의 경우 빠른 회복에 도움을 줄 것으로 예상할 수 있다. 둘째, 사해염을 단독으로 도포하였을 경우 세라마이드를 단독 도포한 경우보다 장벽 회복률이 다소 높았으나 통계학적으로 유의하지는 않았다.
이와 같이 손상된 피부장벽회복에 대한 사해염의 직접적인 영향은 아직 알려져 있지 않으나 사해염이 세라마이드와 유사한 피부장벽 회복률을 보였다. 특히 사해염과 세라마이드를 혼합하여 사용할 경우 피부장벽 회복률이 현저히 높았다. 이는 급성 또는 만성적인 장벽손상이 나타나는 피부 질환 치료로 사해염과 세라마이드 혼합 크림이 더 효과적으로 임상에 적용할 수 있을 것임을 시사한다.
Keratinized lipid is made of 26% of ceramide which is one of the main lipid components of keratin occupying the space between the keratinocytes. Keratinized lipid has the function of the skin barrier and moisturizing to the skin. Especially ceramide has main role to the recovery of the skin barrier with acute and chronic dermatologic problems.
Since it is reported that hot spa therapy at dead sea is effective on patients who have psoriasis and atopic dermatitis, lots of dead sea salt-products which contain majority of MgCl2, NaCl, KCl, Br and CaS components are applied to treat patients who have dermatologic problems because it has been known that the salt-products play an important role to prevent the proliferation of squamous cell, to prevent mitosis of epithelial cells, to remove the normal flora on the skin and to block the antigenic expression of the keratinocytes though the exact pathophysiologic mechanism of action is not fully understood. On the basis of these backgrounds, plenty of dead sea salt-products, including moisturizer which consists of dead sea salts are being developed and applied to the patients with dermatologic disease as an adjuvant therapy recently. But whether the salt-products which are main component of moisturizer have a direct effect on barrier recovery after disruption of skin barrier is not well known yet.
We studied hairless mice whose backs were divided into 5 parts and then different types of cream were applied. These are control, base cream, dead sea salt (1%) cream, ceramide type 3 (2%) cream and mixture of ceramide type 3 (2%) and dead sea salt (1%). Their skin barrier damage was carried out by scotch tape method and then transepidermal water loss (TEWL) was measured right after tape stripping, 2.5, 6, 10, 24 and 48 hours.
And, we compared the effect of ceramide with that of dead sea salts on the recovery of the skin barrier after acute disruption of skin barrier. Those results are as follows. First, the skin barrier recovery rate of mixture group of dead sea salts and ceramide type 3 is significantly higher than both dead sea salts alone and ceramide alone group, and therefore we suggest mixture type of dead sea salts and ceramide type 3 could help in the rapid recovery of lesion of the patients with acute or chronic skin barrier damage. Second, Skin barrier recovery rate of only dead sea salt group is higher than only ceramide group but there was no statistically significant correlation.
In summary, although the direct effect of dead sea salts for the recovery of disrupted skin barrier is not known, the effect of dead sea salts is similar to that of ceramide in the recovery rate of skin barrier, and the mixed cream of dead sea salts and ceramide has a higher recovery rate of skin barrier than other groups.
In conclusion, this study suggests that the mixture cream of dead sea salt and ceramide could be clinically more effective for the treatment of dermatologic diseases with presentation of acute or chronic disruption of skin barrier.
