간호대학생의 임상실습 스트레스, 스트레스 대처 및 공감능력이 임상수행능력에 미치는 영향

Abstract

The purpose of this study was to identify the effects of clinical practice stress, stress coping and empathy on clinical competency among nursing students, and to analyze difference of affecting factors on the level of clinical competency. Data were collected from 201 senior nursing students from 3 universities in D city. The survey was conducted from June 15th, 2016 to August 1st, 2016. Data were analyzed by descriptive statistics, t-test, ANOVA, Pearson’s correlation coefficient, Multiple linear regression and Quantile regression using SPSS 21.0 and STATA 14.0 program. The result of this study were as follows: 1) In the general characteristics of nursing students, clinical competency was associated with motives of application to nursing major(F=2.75, p=.029), nursing major satisfaction(F=5.70, p<.001), clinical practice satisfaction(F=4.88, p<.001), grade(F=5.90, p<.001), practice peer relationships(F=7.87, p<.001) and difficult relationships in practice(F=3.44, p=.018). 2) Clinical competency showed positive correlation with clinical practice stress related to practice workload(r=.23, p=.001), active stress coping(r=.63, p<.001), passive stress coping(r=.24, p=.001), cognitive empathy(r=.40, p<.001) and emotional empathy(r=.27, p<.001); negative correlation with clinical practice stress related to interpersonal relationships(r=-.18, p=.012). 3) The affecting factors of clinical competency were clinical practice stress related to clinical learning environment(β=-.18, p=.007), clinical practice stress related to practice workload(β=.22, p<.001) and active stress coping(β=.57, p<.001), and these variables explained 50% in clinical competency. 4) After quantile regression, affecting factors of clinical competency were active stress coping for all quantiles, clinical practice stress related to clinical learning environment for from 15% quantile to 60% quantile, clinical practice stress related to practice workload for from 15% quantile to 75% quantile and nursing major satisfaction for 15% quantile. This study indicated that affecting factors of clinical competency among nursing students were different from the level of clinical competency. Specially, the following should be considered in the improvement of clinical competency for the low level group students. Increasing nursing major satisfaction, managing clinical practice stress related to clinical learning environment, imposing proper clinical practice stress related to clinical workload and strengthening active stress coping. The results of this study can be used as the basic data for the customized intervention strategy for improvement of clinical competency considering the level of clinical competency among nursing students.

본 연구는 간호대학생의 임상실습 스트레스, 스트레스 대처 및 공감능력이 임상수행능력에 미치는 영향을 파악하고, 분위별 임상수행능력 수준에 따른 영향요인의 변화를 분석하기 위하여 실시되었다. 자료수집은 D광역시에 소재한 3개 대학의 4년제 간호학과에 재학 중인 4학년 학생을 대상으로 조사하였으며, 2016년 6월 15일부터 2016년 8월 1일까지 진행되었다. 자료분석은 SPSS 21.0 프로그램과 STATA 14.0 프로그램을 이용하여 기술통계, t-test, ANOVA, Pearson’s correlation coefficient, 다중회귀분석, 분위회귀분석을 사용하였다. 연구결과는 다음과 같다. 1) 간호대학생의 일반적 특성에 따른 임상수행능력은 입학동기(F=2.75, p=.029), 전공만족도(F=5.70, p<.001), 임상실습만족도(F=4.88, p<.001), 학업성적(F=5.90, p<.001), 실습동료와의 관계(F=7.87, p<.001), 실습 시 어려웠던 관계(F=3.44, p=.018)에 따라 유의한 차이가 있었다. 2) 간호대학생의 임상수행능력은 실습업무 부담 관련 임상실습 스트레스(r=.23, p=.001), 적극적 스트레스 대처(r=.63, p<.001), 소극적 스트레스 대처(r=.24, p=.001), 인지적 공감(r=.40, p<.001), 정서적 공감(r=.27, p<.001)과 유의한 양의 상관관계를 보였으며, 대인관계 갈등 관련 임상실습 스트레스와 유의한 음의 상관관계(r=-.18, p=.012)를 보였다. 3) 간호대학생의 임상수행능력에 영향을 미치는 요인은 실습교육 환경 관련 임상실습 스트레스(β=-.18, p=.007), 실습업무 부담 관련 임상실습 스트레스(β=.22, p<.001)와 적극적 스트레스 대처(β=.57, p<.001)로 나타났으며, 이들 변수의 임상수행능력에 대한 설명력은 50%로 나타났다. 4) 간호대학생의 분위별 임상수행능력에 영향을 미치는 요인은 적극적 스트레스 대처는 전 분위에서, 실습교육 환경 관련 임상실습 스트레스는 15%에서 65% 분위에서, 실습업무 부담 관련 임상실습 스트레스는 15%에서 75% 분위에서, 전공만족도는 15% 분위에서 유의한 것으로 나타났다. 본 연구결과를 통해 간호대학생의 임상수행능력 수준에 따라 각각의 영향요인이 다른 것을 알 수 있었다. 특히, 하위그룹 학생들의 임상수행능력 향상을 위해서는 전공만족도 증진, 실습교육 환경 관련 스트레스 관리, 적절한 실습업무 부담, 적극적 스트레스 대처 강화가 고려되어야 한다. 이러한 연구결과는 간호대학생의 임상수행능력 수준을 고려한 맞춤형 임상수행능력 향상 전략의 기초자료로 활용될 수 있을 것이다.
