Abstract
The purpose of this study was to identify the factors influencing on the nurses' attitude about advance directives. In order to identify these factors, a cress-secrional survey was adopted. The survey was carried out from August 1 to August 30, 2016, the data were collected from D hospital in D city. A total of 196 nurses participated in this study. The instruments used for this study were questionnaires to measure the nurses' attitude, experience, knowledge and perception of good death. The data were analysed with frequency, percentage, mean, standard deviation, t-test, ANOVA, Pearsonʼs correlation coefficients, multiple regression analysks by using SPSS/WIN program version 22.0. The results of this study were as follows: The mean score of attitudes for advance directives was 29.36±0.52, participants' experience score showed 2.48±1.76, knowledge for advance directives was 7.60±1.31, perception of good death 47.73±5.54. Based on the nurse' general characteristics, work characteristics and death characteristics of subject, the attitude score showed a significant difference in terms of age (F=3.97, p=.020), gender (t=2.15, p=.033), marital status (t=-2.40, p=.018), status of offspring (t=2.98, p=.003), educational background (t=-3.08, p=.002), working experience (F=7.47, p=.001), time of documenting written advance directives (F=3.64, p=.007), experience of death in the family members (t=2.44, p=.015), and importance of religion (F=5.04, p=.002). There were significant positive correlations between attitude and experience of advance directive (r=.15, p<.001); and between attitude and perception of good death (r=.37, p<.001). The significant influencing factors on attitude for advance directive were perception of good death (β=.32, p<.001) and time of documenting written advance directives (β=.25 p=.025), explaining 20% of the variance in the multiple regression model. Therefore, in order to improve the nurses' attitude for advance directives, it is necessary to develop strategies and educational programs for nurses and in a clinical setting.
본 연구는 임상간호사를 대상으로 사전의료의향서에 대한 태도에 영향을 미치는 요인을 파악하고자 실시되었다. 본 연구는 횡단적 조사 연구이다. 본 연구의 자료 수집 기간은 8월 1일부터 8월 31일까지이며, 연구 대상자는 D광역시 소재 1개 대학병원 임상간호사 196명을 대상으로 하였다. 연구도구는 사전의료의향서에 대한 태도, 경험, 지식 및 좋은죽음인식에 관한 도구를 사용하였으며, 자료분석은 SPSS/WIN 22.0 통계프로그램을 이용하여 빈도분석, 백분율, 평균과 표준편차, t-test, ANOVA, Pearson correlation coefficients, multiple regression analysis를 사용하였다. 다음의 결과를 도출하였다. 임상간호사의 사전의료의향서에 대한 태도점수는 29.36±0.52점, 경험점수는 2.48±1.76점, 지식 점수는 7.60±1.31점, 좋은죽음인식 점수는 47.73±5.54점으로 나타났다. 임상간호사의 일반적, 근무관련 및 죽음관련 특성에 따른 사전의료의향서에 대한 태도 차이에서 연령(F=3.97, p=.020), 성별(t=2.15, p=.033), 결혼상태(t=-2.40, p=.018), 자녀유무(t=2.98, p=.003), 교육정도(t=-3.08, p=.002), 근무경력(F=7.47, p=.001), 사전의료의향서 작성시기(F=3.64, p=.007), 가족죽음 경험(t=2.44, p=.015), 종교의 중요성(F=5.04, p=.002)에서 유의한 차이가 나타났다. 사전의료의향서 태도 관련 경험(r=.15, p<.001), 좋은죽음인식(r=.39, p<.001)에서 통계적으로 유의한 정적인 상관관계가 있었다. 대상자의 사전의료의향서에 대한 태도에 영향을 미치는 요인을 분석한 결과, 좋은죽음인식(β=.32, p<.001), 사전의료의향서 작성 시기(β=.25, p=.025)이 영향요인으로 나타났고, 전체 설명력은 20%였다. 결론적으로, 본 연구를 토대로 임상간호사의 사전의료의향서의 작성에 대한 태도를 증진시키기 위한 좋은죽음교육 프로그램이 요구되며, 적절한 전략이 개발 및 제공되어야 할 것이다.
