중환자실 간호사의 연명의료에 대한 역할인식, 간호스트레스 및 태도

Abstract

The purpose of this study was to identify the relationships between role perception, nursing stress and attitude on life sustaining treatment among intensive care unit nurses. The data collection was made from the 166 nurses who have experience of nursing the patients receiving life sustaining treatment in intensive care units of general hospitals of over 300 beds located in 3 megalopolises and 1 province between December 1, 2015 and January 31, 2016. In order to measure role perception, the tool developed by Byun et al was used. The tool for nursing stress on life sustaining treatment was developed through the preliminary research, expert content validity test and reliability test. For measuring subject's attitude on life sustaining treatment, the tool developed by Byun et al was used. The data analysis was performed using the frequency, percentage, average, standard deviation, t-test, ANOVA, Scheffe's test and Pearson's correlations coefficient using the SPSS Window IBM 21.0 program. Results of the study were as follows : The mean score of role perception on life sustaining treatment was 3.95±0.51, nursing stress 3.73±0.51, and attitude 3.32±0.45. According to general characteristics, the role perception on life sustaining treatment showed significant differences depending on educational background (t=-2.19, p=.030), nursing career (F=5.03, p=.002), work department (F=4.03, p=.008) and necessity of advance directives (t=2.23, p=.027). For subjects' nursing stress on life sustaining treatment, there were significant differences depending on gender (t=-2.26, p=.025), educational background (t=-3.37, p=.001), nursing career (F=2.90, p=.037), work department (F=3.93, p=.010) and the experience of life sustaining treatment of their family members or relatives within in the past year (t=2.33, p=.021). There were significant positive correlations between role perception and nursing stress on life sustaining treatment (r=.22, p=.004) and between role perception and the attitude on life sustaining treatment (r=.21, p=.007). In conclusion, the higher the role perception on life sustaining treatment among ICU nurses, the higher the nursing stress and attitudes were reported. Based on the result of this study, the nursing education program related to the life sustaining treatment for effective nursing life sustaining treatment and nursing strategy to reduce the nursing stress from life sustaining treatment of the nurses in intensive care units need to be developed afterward.

본 연구는 중환자실 간호사의 연명의료에 대한 역할인식, 간호스트레스 및 태도를 알아보고 이들 간의 관계를 파악함으로써 효율적인 연명의료간호를 위한 기초자료를 제공하고자 시도되었다. 자료수집은 2015년 12월 1일부터 2016년 1월 31일까지 3개의 광역시와 1개의 도에 소재한 300병상 이상 종합병원 중환자실에서 연명의료 환자를 간호한 경험이 있는 간호사 166명을 대상으로 하였다. 연명의료에 대한 역할인식은 Byun 등이 개발한 도구를 이용하여 측정하였고, 연명의료 간호스트레스 측정도구는 본 연구자가 개발한 도구로서 문헌고찰과 심층면담을 거쳐 문항을 구성한 후 전문가를 통한 내용타당도와 요인분석을 통한 구성타당도를 확인하였다. 연명의료에 대한 태도는 Byun 등의 도구를 수정·보완하여 사용하였다. 자료분석은 SPSS Window IBM 21.0 프로그램을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, ANOVA, Scheffe's tset 및 Pearson’s correlation coefficient로 분석하였다. 연구결과 연명의료에 대한 역할인식 정도는 평균 3.95±0.51점, 연명의료 간호스트레스 정도는 3.73±0.51점, 연명의료에 대한 태도 정도는 3.32±0.45점으로 나타났다. 대상자의 특성에 따른 연명의료에 대한 역할인식은 최종학력(t=-2.19, p=.030), 임상간호경력(F=5.03, p=.002) 현재근무부서(F=4.03, p=.008), 사전연명의료의향서 필요성(t=2.23, p=.027)에 따라, 연명의료 간호스트레스는 성별(t=-2.26, p=.025), 최종학력(t=-3.37, p=.001), 임상간호경력(F=2.90, p=.037), 현재근무부서(F=3.93, p=.010), 최근 1년 이내 가족, 친척의 연명의료경험(t=2.33, p=.021)에 따라 유의한 차이가 나타났다. 연명의료에 대한 역할인식, 간호스트레스 및 태도 간의 상관관계에서 역할인식은 간호스트레스(r=.22, p=.004)와 태도(r=.21, p=.007)에 각각 유의한 양의 상관관계가 있었다. 본 연구결과를 바탕으로 향후 효율적인 연명의료 간호를 위한 교육 프로그램과 중환자실 간호사들의 연명의료 간호스트레스를 감소시킬 수 있는 간호전략 개발이 필요하다.
