초기폐경기후의 여자에서 혈중 LH; FSH 및 Estradiol-17 β의 농도와 그 증상에 관한 연구

Abstract

The frequencies of climacteric symptoms of Kupperman index, serum concentrations of LH, FSH and estradio-17βand the significance of difference in serum levels of LH, FSH and estradiol-17βin postmenopausal women with and without climacteric symptoms were studied in 36 early post-monopausal women(within 1 year after last menstrual bleeding) who visited to internal medical outpatient clinic of Keimyung Unversity Dongsan Medical Center from Jan. 1985 to Oct. 1985. The results were summarized as follows: 1. Among climacteric symptoms of Kupperman index in early postmenopausal women, the commonest is fatigue (86%) and the nexts were depressions. vertigoes and arthralgias myalgias (75% respectively), hot flushes and palpitations (69% respectively), nervousnesses(67%), heada-ches(64%), paresthesias(58%), insomnias(44%) and foamication (33%). 2. The mean serum concentrations of FSH, LH and estradiol-17βin early postmenopausal women were 73.4±30.7 mIU/ml, 45.3±24.2 mIU/ml, 51.2±30.3Pg/ml respectively which were much higher serum concentrations of LH and FSH and 50% lower levels of estradiol-17βthan premeno-pausal women. The increase in serum concentrations of FSH is more prominent than in LH. 3. Serum concentrations of FSH and LH were significantly (p＜0.001. p＜0.002 respectively) higher and levels of estradiol-17βwere significantly (p＜0.005) lower in women complaining of hot flushes than in women without. Serum levels of LH were significantly (p＜0/001) higher and levels of estradiol-17βwere significantly (p＜0.05) lower than in women without and women with fatigue had significantly (p＜0.05) lower estradiol-17βlevels than women without.

저자들은 1985년 1월부터 1985년 10월까지 계명대학교 동산의료원 내과에 내원한 폐경기후 1년이내 이면서 Kupperman index 증상을 호소하는 36명의 환자에서 증상의 빈도, FSH, LH 및 estradiol-17β의 혈중농도, 증상과 호르몬농도들의 관계를 관찰하여 다음과 같은 결론을 얻었다. 1. 초기폐경후 Kupperman index 증상증 피로감(86%)이 가장 많았고 우울증, 현훈 및 관절통/근육통(각각 75%), 열홍조 및 심계항진(각각 69%), 신경질(67%), 두통(64%), 감각이상(58%), 불면증(44%), 의주감(33%) 등의 순이었다. 2. 초기폐경기후의 혈증 FSH, LH 및 estradiol-17β의 평균농도는 각각 73.4±30.7mIU/㎖, 45.3±24.2mIU/㎖, 51.2±30.2pg/㎖로 FSH와 LH는 가임기여성보다 훨씬 증가했으며 특히 FSH 혈중농도증가가 현저했고 estradiol-17β 농도는 가임기여성보다 50%이상 감소했다. 3. 초기폐경기후 혈중 FSH, LH 및 estradiol-17β농도와 Kupperman index 증상과 관계는 열홍조있는 환자에서 없는 환자에 비해 FSH와 LH혈중농도가 의미있게 (각각 p＜0.001, p＜0.002) 증가했고 estradiol-17β혈중농도는 의미있게 (p＜0.005) 감소했으며 불면증있는 환자에서 없는 환자에 비해 내혈중농도증가(p＜0.001)와 estradiol-17β농도감소 (p＜0.05)가 통계적으로 의미있고 피로감 있는 환자에서 없는 환자에 비해 estradiol-17β혈중농도가 의미있게 (p＜0.05) 감소했으나 다른 증상들과 혈중호르몬 농도들과는 무관하게 나타났다.
