원위요골 골절에서 수장 측 금속판 제거술은 필요한가

Abstract

This study evaluated the clinical outcome of before and after of plate removal in distal radius fracture, compared with control group who did not removed the plate. In Dongsan hospital of Keimyung University, among the 104 case of distal radius fracture, study group, 36 patients whose removed volar plate, and control group, 36 patients whose do not removed volar plate after union of distal radius fracture were selected during January 2011 to December 2015. Q-DASH and Mayo wrist score were used to compare the clinical outcome. In study group, Q-DASH score improved from 21.6 to 14.7, Mayo wrist score improved from 75.8 to 88.6. In control group, Q-DASH changed from 20.2 to 20.6, Mayo wrist score changed from 82.5 to 81.5. Compare with study group and control group clinical score got significant change in both Q-DASH and Mayo wrist score. Patients who complain of discomfort or less satisfied after volar plate fixation in distal radius fracture, may consider removal of volar plate after bony union.

수장 측 금속판 고정술을 시행한 원위 요골 골절에서 금속판 제거술 전 후의 임상 결과를 비교하고 또 이를 금속판을 제거하지 않은 대조군과 비 교하여 금속판 제거술의 필요성에 대해 연구하고자 하였다. 계명대학교 동 산병원에서 2011 년 1 월부터 2015 년 12 월까지 원위 요골 골절 진단하에 수장측 금속판 내고정술을 시행한 104 예 중 기구 제거술을 시행하고 6 개 월 이상 추시 관찰이 가능하였던 36 예의 환자들을 연구군 으로 정하고 골 유합 후 기구 제거술을 시행하지 않고 6 개월 이상 추시관찰이 가능하며 연구군과 성별, 나이, 골절양상이 비슷한 36 예의 환자들을 대조군으로 정 하여 후향적 으로 연구 하였다. 임상적 결과 평가는 Quick-disabilities of the arm, shoulder and hand(Q-DASH) 점수와 Mayo wrist 점수를 비교하 여 평가하였다. 연구군에서 Q-DASH 점수는 제거술 전 21.6 점에서 제거술 후 14.7 점으로 호전을 보였고 Mayo wrist 점수는 제거술 전 75.8 점에서 제거술 후 88.6 점으로 호전을 보였다. 대조군에서 Quick-DASH 점수는 골 유합 직후 평균 20.2 점에서 최종 추시시 평균 20.6 점으로 임상적 결과가 유의한 차이가 없었고 Mayo wrist 점수는 골유합 직후 평균 82.5 점에서 최종 추시시 평균 81.5 점으로 유의한 차이가 없었다. 연구군과 대조군 간 의 Quick-DASH 점수 변화 차이는 연구군이 6.89 점 호전, 대조군이 0.47 점 악화 로 두 군 간의 유의한 차이가 있었으며 Mayo wrist 점수의 변화 값의 차이는 연구군이 12.78 점 호전, 대조군이 0.97 점 악화로 두 군 간의 유의한 차이가 있었다. 금속판 고정술 후 지속적으로 수장측의 불편감을 호 소하거나 술 후 만족도가 낮은 환자들에서는 골유합 이후 금속판 제거술을 고려할 수 있을 것으로 사료된다
