Clinical Characteristics of Patients of Patients Who Maintained Peritoneal Dialysis Over a Decade

Abstract

The peritoneal dialysis (PD) is one of the major modalities of renal replacement therapy in end-stage renal disease. However, PD is declining lately in prevalence and incidence. To analyze the characteristics of patients who safely maintained long-term PD, this study retrospectively compared 76 patients who had maintained PD over 10 years to 109 patients who had failed PD within 1 year. Compared to early failure group, the mean age at the initiation of PD in the long-term group was younger. The proportion of patients with diabetes as etiology of end-stage renal disease was smaller, and the incidence of PD peritonitis was lower. The mean time from PD start to first PD peritonitis was longer. The independent risk factors associated with technical failure were female gender, concomitant cardiovascular disease, PD peritonitis rate, high hemoglobin calcium levels. The characteristics of patients who maintained long-term PD were younger at the start of PD, less diabetic, late onset of first PD peritonitis, and less PD peritonitis, showing significant difference compared to early failure group. To maintain PD for long time safely, we should pay more attention to maintain low incidence of PD peritonitis, especially in old age and diabetes patients.

복막투석은 말기 신부전에서 중요한 신 대체 요법 중의 하나이나, 기술적 문제 및 복막염 등의 합병증으로 인해 그 빈도가 감소하고 있다. 그럼에도 불구하고 장기간 복막투석을 효과적으로 유지하고 있는 환자들이 있으며, 복막투석이 혈액투석에 비해 보다 생리적인 투석 기법이고 생존율에 긍정 적인 영향을 미친다는 연구가 있다. 따라서, 본 연구에서는 장기간 복막 투 석을 유지하는 환자들의 공통된 소인을 분석하기 위해 복막투석을 10년 이 상 유지한 환자 76명과 1년 이내에 복막투석을 중단한 109명을 후향적으로 비교하였다. 복막투석을 조기에 실패한 환자군과 비교하여, 장기간 복막투 석을 유지했던 환자군은 투석 시작 연령이 낮았고, 당뇨 유병률이 더 적었 다. 또한 복막염 발생률이 더 낮았으며 첫번째 복막염 발병까지의 기간이 더 길었다. 환자 사망을 제외한 투석 유지 실패의 독립적인 위험인자는 여 성, 동반된 심혈관 질환, 복막염의 빈도, 그리고 높은 헤모글로빈 및 칼슘 수치였다. 따라서, 복막 투석을 장기간 안전하게 유지하기 위해서는 복막염 의 예방이 가장 중요하며, 특히 당뇨 및 고령 환자들의 복막투석 치료에서 더욱 각별한 주의가 요구된다.
