dc.description.abstractThis research was performed to determine the factors influencing on hyperphosphatasis of patients with chronic hemodialysis. The subjects of the study were 129 patients who had been diagnosed with late stage of renal disease and received two or three times of hemodialysis per a week. Data were collected from 15 th June, 2017 to 15 th August, 2017. Data were analyzed with frequency, percentage, average, standard deviation, t-test, χ²-test, Logistic regression using SPSS WIN 24.0 version program. The results of this study were as follows: 1. Of these 129 hemodialysis patients, 71 patients (55%) were hyperphosphatasis. 2. Initially univeariate analysis was conducted to identify candidate predicInitially univariate analysis was conducted to identify candidate predictor variablesor variables that were BMI(p=.004), times of hemodialysis per a week (p=.024), TIBC (p=.035), Ca×P (p<.001), Creatine (p=.012), subjective nutritive conditions(p=.024), weight increase between hemodialysis (p=.001), BUN (p<.001), age (p=.048), causes disease of chronic renal failure (p=.026), diet (p=.035). 3. In the multivariate analysis, the number of hemodialysis patients who had hyperphosphatasis were significantly increased with under common the following co-factors: diabetes(CI=1.38-333.47, OR=21.45), hypertensives(CI=1.15-180.12, OR=14.41), subjective nutritive conditions (CI=1.36-64.18, OR=9.37), weight increase between hemodialysis (CI=1.18-20.05, OR=4.86), BUN (CI=1.01-1.14, OR=1.07). Consequently, it is found that hypertension, diabetes, nutritive condition, weight fluctuation, and BUN need to be reflected as important factors on managing hyperphosphatemia among hemodialysis patients since those are identified as important predicting factors. importance of hyperphosphatemia among hemodialysis patients need to be recognized through this study. I expect this study can be important basic data on development of integrated nursing intervention for intervention of hyperphosphatemia and risk of hyperphosphatemia on hemodialysis patients goes lower.-
dc.description.abstract본 연구는 혈액투석 환자의 고인산혈증 발생에 대한 예측요인을 파악하 기 위해 시도된 후향적 코호트 연구이다. 연구대상은 D광역시 소재 K대학 병원에 만성신부전으로 진단받고 주2-3회 혈액투석을 실시하는 환자 129명 을 대상으로 하였고, 자료 수집은 2017년 6월 15일부터 2017년 8월 15일까 지 이루어졌다. 자료 분석은 SPSS WIN 24.0 Program을 이용하여 빈도, 백분율, 평균, 표준편차, t-test, χ²-test, Logistic regression을 이용하여 분석하였고, 본 연구의 결과는 다음과 같다. 1. 혈액투석 환자 129명 중 고인산혈증은 71명(55%)로 나타났다. 2. 고인산혈증에서 통계적으로 유의하게 나타난 변수들은 BMI (p=.004), 주당 투석 횟수(p=.024), TIBC (p=.035), Ca×P (p<.001), Creatine (p=.012), 주관적 영양상태(p=.024), 투석 간 체중 증가(p=.001), BUN (p<.001), 연령 (p=.048), 만성신부전 원인 질환(p=.026), 식이(p=.035)이었다. 3. 로지스틱 회귀분석 결과 당뇨(CI=1.38-333.47, OR=21.45), 고혈압 (CI=1.15-180.12, OR=14.41), 주관적 영양상태(CI=1.36-64.18, OR=9.37), 투 석 간 체중 증가(CI=1.18-20.05, OR=4.86), BUN (CI=1.01-1.14, OR=1.07)이 유의한 예측요인으로 확인되었다. 결론적으로 혈액투석 환자의 만성신부전 원인 질환 중 고혈압, 당뇨와 영 양상태 중 주관적 영양상태, 투석 간 체중 증가, BUN은 고인산혈증 발생의 중요한 예측요인임이 확인되었으므로 혈액투석 환자의 고인산혈증을 관리 함에 있어 이러한 요인들을 반영해야 할 것이다. 따라서 본 연구 결과를 통 해 혈액투석 환자의 고인산혈증에 대한 중요성을 인식하고 임상에서 고인 산혈증을 중재하기 위한 통합적 간호중재 개발의 중요한 기초자료로 사용 되어 혈액투석 환자의 고인산혈증 발생 위험이 줄어드는 계기가 되기를 기 대한다.-
