Abstract
The purpose of this study is to develop evidence-based nursing
practice guidelines for pain management in dementia and verify its
effects.
The guidelines were developed according to the Clinical Practice
Guideline Adaptation Manual Ver. 2.0 (Kim et al., 2011). The final
practice guidelines consisted of recommendations for nursing assessment,
intervention, and the health care system. This study was based on the
Diffusion Theory of Innovation and consists of development, education,
acceptance, and outcome according to the Practical Implication
framework of the Clinical Practice Guidelines.
The study design was based on a methodological design model, and it was a nonequivalent control group pretest-posttest design, along with
guideline development. Data was collected from June to August 2017.
The research subjects included two geriatric hospitals in B city and one
in U city in Korea. There was one experimental group and two control
groups, 30 nurses and 28 elderly patients with dementia participated at
one geriatric hospital in B city in the experimental group, and the
control group consisted of 27 nurses and 29 elderly patients with
dementia at two geriatric hospitals in B city and U city. Three
education sessions and four weeks of application were conducted.
The effect of the practical guidelines was verified by the nurses'
knowledge, attitude, pain assessment frequency, pharmacological &
nonpharmacological intervention, and patient pain scores. Data was
analyzed using descriptive statistics, Kolmogrov Smirnov normality test,
chi-square, Fisher's exact test, independent t-test, Mann-Whitney U
test, Wilcoxon Signed Ranks, and ANCOVA test with SPSS 22.0
version.
The findings were as follows: 1. The developed practice guidelines consisted of 107 recommendations:
38 for nursing assessment; 62 for intervention, and 7 for the health care
systems.
2. The guidelines application and effect verification results are as
follows:
1) The first hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have a higher degree of knowledge about
pain in dementia than the control group.” Hypothesis 1 was accepted
(Z=-2.20, p=.027).
2) The second hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have a higher degree of attitudes about pain
in dementia than the control group.” Hypothesis 2 was accepted
(t=-4.13, p<.001).
3) The third hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in patients with dementia will have a higher frequency of pain assessment
than the control group.” Hypothesis 3 was accepted (F=6.60, p=.017).
4) The fourth hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have more days to take analgesics Around
the Clock than the control group.” Hypothesis 4 was accepted (Z=-3.60,
p<.001).
5) The fifth hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will increase the analgesics dose (MQS) more
than the control group.” Hypothesis 5 was accepted (F=4.67. p=.035).
6) The sixth hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have more nonpharmacological intervention
therapy than the control group.” There were no significant differences in
oriental treatment (Z=-1.17, p=.076) and etc. (Z=-0.47, p=.637). But
physical therapy and relaxation (F=17.14, p<.001) have significant differences compared to the control group. Hypothesis 6 was partially
accepted.
7) The seventh hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have a less self-reported pain scale score
than the control group.” There were no significant differences in NRS
(r) (F=0.01, p=.915), NRS (m) (F=0.00, p=.929), FPS (r) (F=1.95, p=.168),
FPS (m) (F=1.51, p=.225) compared to control group. Hypothesis 7 was
rejected.
8) The eighth hypothesis: “The experimental group that applied for
evidence-based nursing practice guidelines for pain management in
patients with dementia will have a less observational pain assessment
scale score than the control group.” There were no significant
differences in PAINAD (r) (F=1.93, p=.171), PAINAD (m) (F=1.16,
p=.285) compared to the control group. Hypothesis 8 was rejected.
In conclusion, the application of the evidence-based nursing practice
guidelines for pain management in dementia has been found to be effective in increasing the nurses' knowledge, attitude, pain assessment
frequency as well as pharmacological and nonpharmacological
intervention. Therefore, it is necessary to actively measure to ensure
continued use of these practice guidelines
본 연구는 치매환자의 통증관리를 위한 근거중심 실무지침을 개발하고,
이를 적용하여 그 효과를 검증하기 위한 목적으로 실시하였다.
실무지침은 임상진료지침 수용개작 매뉴얼 Ver 2.0(김수영 등, 2011)에
제시된 방법을 활용하여, 국외에서 개발된 치매환자 통증관리 가이드라인을
국내실정에 맞게 개발하였다. 최종 확정된 실무지침은 간호사정을 위한 권
고, 중재를 위한 권고, 건강관리체계에 대한 권고로 이루어졌다. 본 연구는
혁신확산 이론을 기본으로 한 임상실무 가이드라인 실무적용 개념 틀에 따
라 개발, 교육, 수용, 결과로 구성되었다.
연구설계는 비동등성 대조군 전후설계이며, 자료수집기간은 2017년 6월
부터 2017년 8월까지 이루어졌다. 연구대상은 B광역시 노인요양병원 2곳,
U광역시 노인요양병원 1곳을 대상으로 실험군 1곳, 대조군 2곳을 선정하였
으며, 간호사 실험군 30명, 대조군 27명과 65세 이상 치매환자 실험군 28명, 대조군 29명이었다.
실무지침 적용효과는 간호사의 지식, 태도, 통증사정빈도, 약물적중재,
비약물적중재, 환자의 통증점수로 검증하였다. 자료분석은 SPSS 22.0
version을 이용하여, 정규성 검정은 Kolmogorov Smirnov 검정, 사전 동질
성검증은 Chi-square test, Fisher's exact test, independent t-test, 가설검
증은 independent t-test, Mann-Whitney U test, Wilcoxon Signed Ranks,
ANCOVA, 신뢰도 검사는 Cronbach's α를 이용하였다.
연구결과는 다음과 같다.
1. 개발된 실무지침은 총 107개의 권고로 간호사정을 위한 권고 38개,
중재를 위한 권고 62개, 건강관리체계에 대한 권고 7개로 구성하였다.
2. 실무지침 적용과 효과검증 결과는 다음과 같다.
1) 제 1가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
간호사는 적용하지 않은 대조군 간호사보다 치매환자 통증에 대한 지식이
높을 것이다.”를 검증한 결과 실험군의 지식은 대조군과 비교하여 통계적으
로 유의한 차이가 있었다(Z=-2.20, p=.027). 따라서 제 1가설은 지지되었다.
2) 제 2가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
간호사는 적용하지 않은 대조군 간호사보다 치매환자 통증관리태도 점수가 높을 것이다.”를 검증한 결과 실험군의 태도는 사정태도(t=-2.87, p=.006),
관리태도(t=-4.10, p=<.001)로 대조군과 비교하여 통계적으로 유의한 차이
가 있었다. 따라서 제 2가설은 지지되었다.
3) 제 3가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
간호사는 적용하지 않은 대조군 간호사보다 통증사정빈도가 높을 것이다.”
를 검증한 결과 실험군의 통증사정 빈도는 대조군과 비교하여 통계적으로
유의한 차이가 있었다(F=6.06, p=.017). 따라서 제 3가설은 지지되었다.
4) 제 4가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
환자의 진통제 정규처방 일수는 적용하지 않은 대조군 환자보다 많을 것이
다.”를 검증한 결과 실험군의 진통제 정규처방 일수는 대조군과 비교하여
(Z=-3.60, p<.001) 유의한 차이가 있었다.
5) 제 5가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
환자의 진통제 투여용량은 적용하지 않은 대조군 환자보다 많을 것이다.”를
검증한 결과 실험군의 진통제 투여량 MQS는 대조군과 비교하여 유의한
차이가 있었다(F=4.67. p=.035). 따라서 제 5가설은 지지되었다.
6) 제 6가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
환자의 비약물적 통증관리 중재 빈도는 적용하지 않은 대조군 환자보다 높을 것이다.”를 검증한 결과 실험군의 비약물적 통증관리 중재 빈도는 대조
군과 비교하여 물리요법 및 이완(F=17.14, p<.001) 에서 유의한 차이가 있
었으나, 한방치료(Z=-1.17, p=.076), 기타(Z=-0.47, p=.637)에서는 유의한 차
이가 없었다. 따라서 제 6가설은 부분적으로 지지되었다.
7) 제 7가설: “치매환자 통증관리 근거중심 실무지침을 적용한 실험군
치매환자의 자가보고 통증척도 점수는 대조군 치매환자보다 낮을 것이다.”
를 검증한 결과 실험군의 통증점수는 대조군과 비교하여 NRS (r) (F=0.01,
p=.915), NRS (m) (F=0.00, p=.929), FPS (r) (F=1.95, p=.168), FPS (m)
(F=1.51, p=.225)로 유의한 차이가 없었다. 따라서 제 7가설은 기각되었다.
8) 제 8가설: “치매환자 통증관리 근거중심 실무지침을 적용한 치매환
자의 행동반응관찰 통증척도 점수는 대조군 치매환자보다 낮을 것이다.”를
검증한 결과 실험군의 통증점수는 대조군과 비교하여 PAINAD (r)
(F=1.93, p=.171), PAINAD (m) (F=1.16, p=.285)로 유의한 차이가 없었다.
따라서 제 8가설은 기각되었다.
이상의 결과로 볼 때, 치매환자 통증관리 실무지침은 치매환자 통증에
대한 간호사의 지식, 태도, 통증사정 빈도증가, 약물적 통증관리, 비약물적
통증관리 중재에 효과가 있는 것으로 나타났다. 따라서 본 실무지침을 지속적으로 활용할 수 있는 적극적인 방안의 모색이 필요하다.
