복막투석 환자의 입원에 대한 임상적 고찰

Abstract
Purpose: Hospitalization as a measure of morbidity in peritoneal dialysis (PD) patients is mainly related to peritonitis. And the hospitalization rate is expected to decrease as the peritonitis rate has decreased substantially with development of connectology. Yet there is no internal study on hospitalization. We evaluated hospitalization rates, causes and duration of admission of PD patients, and their prognosis. Methods: We retrospectively reviewed the medical records of 414 patients who started and followed up at least three months at the Keimyung university Dongsan Hospital from January 2003 to December 2008.

Results: There were 1,036 admissions in 328 patients during a mean follow-up of 29.9 months. The admission rate was 1.0 per patients-year and hospital days were 17.1 per patients-year. The number of patients admitted once was 102 (31.1%), and more than 5 was 71 (21.7%). The most common cause of hospital admission was peritonitis (36.5%), followed by volume imbalance (13.8%), gastrointestinal disease (6.9%), other infection (6.2%), neurologic disease (5.5%), surgery (5.4%) and cardiac disease (4.3%). Catheter-related problems, including catheter related infection (1.8%) and catheter dysfunction (1.4%) were uncommon. Duration of admission was longest in neurologic disease (18.0土 19.0) and shortest in catheter-related problems (9.3±3). Duration of admission of peritonitis (16.1±8.0) was similar to mean duration. Hospitalization was significantly greater in patients with prior history of peritonitis (p<0.000), and longer duration on PD (p<0.000). There were no significant differences in one and five year patient and catheter survival between hospitalized and non-hospitalized PD patients. Conclusion : Peritonitis remains a major cause of hospitalization in PD patients. To decrease admissions of PD patients, patient education and attention needs to be focused on preventing peritonitis. Also we should pay more attention to prevent multiple admissions due to recurrent peritonitis.

요약
목 적: 복막투석 환자에서 입원은 이들 환자의 이병율에 대한 척도로서 복막염의 발생과 깊은 관련이 있음이 알려져 있다. 그 동안 투석액 교환방법을 포함한 복막투석 수기의 향상으로 복막 염의 빈도가 감소됨에 따라 이로 인한 입원율도 감소될 것으로 예상되나 아직 국내에서는 이들 환자의 입원에 관한 연구가 보 고된 바가 없다. 저자들은 계명대학교 동산병원에서 복막투석 치 료를 새로이 시작한 환자를 대상으로 입원율, 입원 원인 및 기 간, 입원이 예후에 미치는 영향에 대해 조사하여 보고하는 바이 다.

방법:2003년 1월부터 2008년 12월까지 만6년간계명 대학교 동산병원에서 복막투석을 새로 시작하고, 격어도 3개월 이상 추격관찰이 가능하였던 환자 414명을 대상으로 후향격으 로 조사하였다.

결과:평균추격관찰기간은29.9개월 (최단3.2개월,최장 71.7개월)이었으며, 328명의 환자에서 1,036회 입원하여 입 원율은 1.0회/환자-년, 입원기간은 17.1일/환자-년이었다. 입 원 빈도는 1회 입원한 환자가 102명 (31.1%)이었고 5회 이 상 입원한 환자도 기명 (21.7%)에 달했다. 입원 원인으로는 복막염이 378예 (36.5%)로 가장 많았고 그 외 체액량이상 143예 (13.8%), 소화기질환 72예 (6.9%), 기타 감염질환 64예 (6.2%), 신경질환 57예 (5.5%), 외과수술 56예 (5.4 %) 순이었다. 심장 질환은 47예 (4.4%)였으며, 도관관련 합 병증은34예 (3.2%)였다. 입원기간은신경질환이 18.0±19.0 일로 가장 길었고, 도관 기능 이상이 9.3±3.0일로 가장 짧았다. 복막염의 입원 기간은 16.1±8.0일로 전체 평균 입원기간과 비 숫하였다. 입원 환자와 비입원 환자의 연령, 성별, 당뇨병 환자 의 비율에는 차이가 없었으나 복막염의 과거력, 복막투석 치료기 간이 입원 환자에서 유의하게 높았다. 입원 환자와 비입원 환자 의 환자 생존율과 도관 생존율은 유의한 차이가 없었다.

결 론 : 복막투석 환자에서 복막염은 여전히 입원의 주요 원인 이었으며, 앞으로 복막투석 환자의 입원율을 낮추어 이들의 이병 율을 감소시키고, 삶의 질을 향상시키기 위해서는 지속적인 환자 교육을 통한 복막염 및 수분 장애의 예방에 대한 보다 깊은 관심 과 주의를 기울여야 할 것으로 생각되며 특히 재발성 복막염의 발생을 줄이기 위한 노력이 필요할 것으로 생각된다
