Abstract
Objects : This study was designed to examine the factor structure of the Wisconsin Card Sorting Test(WCST) and to compare the scores of WCST in normal controls, schizophrenic, bipolar, and alcoholic patients.Method : Wisconsin Card Sorting Test and Block design and Comprehension subtests of K-WAIS were administered to hospitalized male schizophrenics(N=22), manics(N=16), alcoholics(N=42) diagnosed by DSM-IVcriteria, and normal controls(N=16).Results : Age at evaluation of alcoholics was significantly older than that of three other groups(F=40.23, p<0.005), and educational level of normal controls was significantly higher than that of three other groups(F=19.38, p<0.001). The principal component analysis on scores for 16 WCST variables extracted three factors: Perseveration, Inefficient Sorting, and Nonperseverative Errors. Components of Perseveration and Nonperseverative Errors factors were different to previous factor analysis results. Number of categories achieved, total errors, conceptual level response, and trials to first category were loaded onto Nonperseverative Errors factor. In total subjects, Nonperseverative Errors factor was negatively correlated with Block design subtest of K-WAIS, Perseveration factor was negatively correlated with educational level and Block design and Comprehension subtests of K-WAIS, and Inefficient Sorting factor was negatively correlated with educational level. The Nonperseverative Errors factor scores of schizophrenics were significantly greater than that of the normal controls(F=3.86, p<0.05). The Perseveration and Inefficient Sorting factor scores showed no significant group differences.Conclusions : These results replicate that the WCST variables are composed of three distinct factors and these 3 representative variables are perseverative errors, nonperseverative errors, and failure to maintain set, and suggest that number of categories achieved, total errors, conceptual level response, and trials to first category share the characteristics of Perseveration factor and Nonperseverative Errors factor.
저자들은 정상 대조군, 양극성 장애 환자, 정신분열병 환자 및 알콜리즘 환자들에서의 위스콘신카드분류 검사의 평가 점수에 대한 각 항목을 서로 비교하고 평가 항목에 대한 요인 분석 하고자 본 연구를 시행하였다.
계명대학교 동산병원 정신과에서 입원 치료를 받았던 남자 환자들 중에 DSM-Ⅳ 기준에 의하여 진단된 양극성 장애 환자 15명, 정신분열병 환자 22명 및 알콜리즘 환자 42명과 남자 정상 대조군 16명을 대상으로 위스콘신카드분류검사와 한국 Wechsler 지능검사의 토막 짜기와 이해력 검사를 시행하였다.
인구학적 특성에서 알콜리즘 집단의 연령이 다른 세 집단들에 비하여 유의하게 더 높았으며(F=40.23, P<0.005), 정상 대조군의 교육 수준이 다른 세 집단들에 비하여 유의하게 높았다(F=19.38, p<0.001). 위스콘신카드분류검사의 16개 평가 점수들에 대한 주성분분석에서 3가지 요인이 추출되었는데, 선행 연구들에서와 마찬가지로 비보속 오류, 보속성 및 비효율적 분류 등으로 명명하였다. 완성범주수, 총오류수, 개념형성수준, 첫범주 완성시도수 등은 비효율적 분류 요인에 포함되었으며 이는 선행 연구들의 결과와 차이가 있었다. 전체 대상자에서 비보속오류 요인은 한국판 Wechsler 지능검사의 토막 짜기 소검사 점수와 역상관관계가 있었고, 보속성 요인은 교육 수준과 한국판 Wechsler 지능검사의 토막 짜기 및 이해력 소검사 점수와 역상관관계가 있었고, 비효율적 분류 요인은 교육수준과 역상관관계가 있었다. 비보속오류 요인 점수는 네 집단간에 유의한 차이가 있었는데(F=3.86, p<0.05), 정신분열병 집단이 정상 대조군에 비하여 유의하게 더 높았고 그 외 다른 두 집단과 대조군 사이에는 유의한 차이가 없었다. 보속성 요인 점수와 비효율적 분류 요인 점수는 네 집단간에 유의한 차이가 없었다.
본 연구의 결과는 WCST 평가 항목이 3가지의 특징적인 요인으로 구성되어 있으며 각 요인의 특징을 가장 잘 설명해주는 항목은 보속오류, 비보속오류 및 틀 유지실패 항목임을 확인시켜 주었고, 완성범주수, 총오류수, 개념형성수준 및 첫 번째 범주 완성 시도수 등의 항목은 보속성 요인과 비보속오류 요인 모두의 특성을 공유하고 있음을 제시해 준다.
