정상 소아에서 심초음파로 측정한 좌심실 질량

Abstract

Purpose:Determination ofleftventricularmyocardialmasswith echocardiography is feasible and validated.American Society ofEchocardiography(ASE)issued recommendations for the quantitation of the left ventricle by M-mode echocardiography.The purposeofthisstudyistodeterminethemeanvalueofleftventricularmassinKorean children. Methods:We measured leftventricularmass in 498 subjects(male 289,female 209) usingASE cubemethodwithcorrectioninM-modemethod. Results: 1)The mean value of left ventricular mass corrected by body surface area are respectively:54.87±27.45 gm/m2 in Korean male children,and 49.57±23.16 gm/m2 in Koreanfemalechildren,byechocardiographyinaccordancewiththeASE convention. 2)The leftventricular mass correlated wellwith the leftventricular end-diastolic dimension,interventricularseptalthickness,leftventricularposteriorwallthickness,and leftventricularend-diastolicvolume. Conclusions:These normalvalues shoud provide a valuable database for further studies ofthe developmentofleftventricular mass in patients with various cardiac abnormalities

목 적 :좌심실 비대를 측정함에 있어서 심초음파 검사가 특이도와 감수도가 우수한 것으로 알 려져 있으며 최근에는 심초음파 검사가 보편화 되었고 이에 따라 좌심실 심근 질량 측정에 따른 기준이 필요하게 되었다.심질환이 없는 소아를 대상으로 American Society ofEchocardiography에서 추천한 방법으로 좌심실 질량을 측정하여 소아의 정상범위를 알아보고자 이 연구를 시행하였다. 방 법 :1992년 6월부터 1994년 5월까지 만 2년간 계명대학교 동산병원 소아과에 입원 또는 방문한 소아중 심질환이 없는 498명을 대상으로 심초음파 검사를 실시하여 좌심실 질량을 측정하 여 그 결과를 분석하였다. 결 과 : 1)해부학적 좌심실 질량은 체표면적당(/m2)은 남아가 54.87±27.45gm,여아가 49.57±23.16 gm 이어서 남아와 여아 사이에 통계적 유의성은 있었다(p<0.001). 2)좌심실 질량은 좌심실 이완기 직경,심실중격 두께,좌심실 후벽 두께,좌심실 이완기 용적 과 모두 상관 관계가 있었다. 결 론 :한국 정상 소아를 대상으로 하여 좌심실 질량을 측정하여 정상 범위를 정하여 보았으 며 이 자료가 좌심실 질량 변화에 관한 연구시 기본 자료로 사용될 수 있으리라 사료된다.
