가족 기능이 신생아 체중에 미치는 영향

Abstract
Background: Low birth weight is the most important determinant of the newborns morbidity and mortality and the healthy growth and development. Pregnancy can be seen as a time when the family's bounderies are shifting to accommodate a new member. Accordingly each family member can be affected from altered family function. Therefore we carried out this study to determine the effects of family functions on infant birth weight. Methods: Randomly selected mothers in AnDong General Hospital and SungSo Hospital obstetric clinics who visit for delivery were invited to participate this study from May through June 1994. The mothers perception of family function was assesed with FACESⅢ and collects sociodemographic, biomedical data. 289 mother-infant pair out of 303 complete questionnares(exception congenital anomaly, multiple pregnancy, placenta or uterine anomaly, preterm) were studies. Infant body weight and other variables affecting body weight were compared by family function type. Results: Infant body weight is positively realted with gestational age, maternal BW, weight gain during pregnancy and negatively related with prematurity delivery history, alcohol. 84.8% of family were considered functional and family function averages flexbly separated. There were no statistically significant difference in the analysis between sociodemographic determinants(education level, occupation, religion, income, marrige, smoking et al) and biomedical characteristics(gestational age, prepregnancy weight and weight gain during pregnancy, hypertension) of functional and dysfunctional families. Infant body weight from with dysfunctional family significantly smaller than from whose mother with functional families(P<0.05). Conclusion: Infant birth weight can be influenced by family function or its associated factors and dysfunctional family increases low birth weight also. Family dysfunction should be possible to intervene early in pregnancy to modify risk factors associated with family systems. So that it will be helps decrease low birth weight. 연구배경 : 신생아의 저체중은 신생아 사망률 및 이환률을 높이는 위험인자이며 소아기의 양호한 신체적 지적 발달에 장애를 주는 요소이다. 임신과 출산기는 역할변화로 가족역동의 변화가 생길 수 있으며 가족기능에 따라 가족 구성원들에게 다양한 영향을 줄 수 있다. 가족기능이 신생아 체중에 미치는 영향을 알아보기 위해 본 연구를 실시하였다.

연구방법 : 1994년 5월 1일부터 6월 30일까지 한 도시 두 종합병원에서 분만한 산모 390 명을 대상으로 설문지를 이용하여 FACESⅢ(Family adaptability and cohesion evaluation scales)를 이용한 가족기능과 산모의 사회경제적면과 생의학적인면을 조사하였다. 설문이 완성된 303명중 선천성 기형이나 다태임신,태반이나 자궁의 기형, 조기분만(〈37주 미만)의 경우를 제외한289쌍을 연구대상으로 조사하여 SPSS./PC+를 이용하여 X2 검정과 t-test, Multiple regression으로 검정을 시행하였다.

결과 : 1,289명의 신생아몸무게는3..27±0.44kg이며 재태기간은39..6±1.64주였으며 남아는 52%(151/289)였고 산모 평균나이는 28.6±3..66kg세, 임신전 평균체중 51.42±6..04kg 임신 중 체중 증가는 평균 12.6±3.77kg이었다. 2. FACESⅢ를 이용한 가족기능은 평균적으로 유연하고 분리된 가족유형이었으며, 기능적인 가족이 84.8%(245/289)였고, 결속력이 신생아 체중과 유의한 관계를 보였다(P<0.05)

3. FACESⅢ로 구분한 기능적인 가족의 신생아 체중은 3.29±044kg으로 기능장애 가족의 3.18±0.49kg에 비해 유의하게 무거웠다(P<0.05).
4 산모의 재태기간, 임신전 체중,임신중 체중증가,음주등의 요인이 신생아 체중과 유의한 상관관계가 있었으며 신생아 체중 및 가족기능과 상관관계가 있는 인구사회경제학적 요인은 없었다.
결론:가족기능은 신생아의 체중에 영향을미치며 기능장애 가족에게 저체중비율이 유의하게 높게 나타나는 것으로 보아 가족기능장애가 신생아 저체중을 유발할 수 있는 인자임을 알고 산모의 가족기능에 영향을 미칠 수 있는 여러 요소들을 교정함으로써 저체중아의 출산빈도를 낮추는데 도움을 주리라 생각한다.(가정의학회지 1995 ; 16 : 585~592)
