상완골 간부 개방성 골절 후 발생한 지연성 상완동맥 폐색 - 증례 보고 -

Abstract

Although vascular injury after humeral fracture is very rare, it is a complication that has serious sequelae. It has been associated with proximal humeral fracture or shoulder dislocation in adults and humeral supracondylar fracture in children. However, delayed brachial artery occlusion after humeral shaft fracture has never been reported worldwide. Nevertheless, delayed brachial artery occlusion after humerus shaft fracture has the potential to cause serious complications in the short term as well as long term; therefore, it is essential to provide accurate diagnosis and prompt treatment. We report a case of delayed brachial artery occlusion after humeral shaft open fracture that was successfully treated with early diagnosis as well as effective treatment. 상완골 골절에서 혈관 손상은 그 빈도가 매우 드물지만 심각한 후유증을 유발할 수 있어 정확한 진단과 즉각적인 치료를 요하는 합병증 중의 하나이다. 상완골 근위부 골절 및 탈구 또는 소아 상완골 과상부 골절에서 잘 발생하는 것으로 알려져 있으나 상완골 간부 골절에 의한 혈관 손상은 극히 드물며, 특히 상완골 간부 골절 후 혈전에 의해 발생한 지연성 상완동맥 폐색의 증례는 전 세계적으로 보고된 적이 없다. 이에 저자들은 상완골 간부 개방성 골절 후 3일째 발생한 지연성 상완동맥 폐색 1예를 경험하였고 조기 진단과 동시에 효과적인 치료를 통하여 좋은 결과를 얻었기에 문헌고찰과 함께 보고하고자 한다. 색인 단어: 상완골 간부 골절, 상완동맥, 동맥 폐색, 지연성
