건강한 성인에서 발생한 수막염을 동반한 Streptococcus agalactiae 감염성 심내막염 1예

Abstract

It is well known that newborns and pregnant women typically show higher rates of Streptococcus agalactiae infection. However, the incidence of group B streptococcal (GBS) disease in nonpregnant adults is increasing, particularly in elderly persons and those with significant underlying diseases, such as diabetes, neurologic impairment, and cirrhosis. Skin infections, osteoarticular infections, and pneumonia are commonly associated with GBS, while meningitis and endocarditis are less common. S. agalactiae endocarditis is an uncommon but important condition with a high mortality rate. Several cases of endocarditis due to S. agalactiae have been reported in the literature, but there have been no reports of S. agalactiae meningitis associated with endocarditis in Korea. Here, we describe a case of S. agalactiae meningitis and endocarditis in a 50-year-old man who was successfully treated with intravenous antibiotics and cardiac surgery. S. agalactiae는 주로 신생아, 주산기 감염의 주요 원인균 으로 알려져 왔으나 최근 들어 성인에서 침습성 감염이 증 가하고 있으며 당뇨병, 악성 종양, 신경계 질환 등이 위험인 자로 알려져 있다. S. agalactiae 감염성 심내막염은 비교적 드물지만 전신적 색전증과 광범위한 판막 파괴로 인한 심부 전이 잘 동반되는 경우가 많아 심장수술이 필요한 경우가 많다. 국내에 S. agalactiae 감염성 심내막염에 대한 여러 보 고는 있었으나 수막염이 동반된 예는 보고된 바가 없다. 저 자들은 기저 질환이 없던 50세 남자에서 S. agalactiae에 의 해 감염성 심내막염과 수막염이 발생한 1예를 진단하여 항 생제 투여 및 심장수술을 통해 성공적으로 치료하였기에 문 헌고찰과 함께 보고하는 바이다. 중심 단어: 심내막염; 뇌수막염; Streptococcus agalactiae
