사시수술 후 굴절이상의 변화

Abstract

To investigate the changes in refractive error following strabismus surgery, we stratified a total of 32 exotropic patients ； 11 patients with horizontal recess/resect procedure in one eye； 11 patients with lateral rectus recession in one eye； and 10 patients with medial rectus resection in one eye. The cycloplegic refraction and corneal topography were examined prospectively in each group. We found a significant decrease in the refractive power at 180° meridian resulting in with the rule astigmatism in lateral rectus muscle recess group (p〈0.05) and a significant decrease in the refractive power at 90° meridian resulting in against the rule astigmatism in medial rectus resection group in the first week after surgery (p<0.05). However, there was no statistically significant change due to mixture of the two procedures in recess/resect group. The changes in refractive power in four months after surgery gradually recovered to preoperative state in each group except 90° meridian in medial rectus resection group. These results indicate that the changes in refractive error in one week after strabismus surgery involved steepening of 180° meridian in medial rectus resection group, .and flattening of 180° meridian in lateral rectus muscle recession group. However, there was no statistically significant change due to mixture of the two procedures in recess/resect group.

사시수술 후 굴절이상의 변화를 알기위하여 수술 방법 즉 단안 내직근절제와 외직근후전을 시 행한 11명, 단안 외직근만 후전율 시행한 11명, 단안 내직근만 절제를 시행한 10명 총 32명의 외사시환자를 대상으로하여 조절 마비 굴절 검사를 술전후에 전향적으로 실시하였다. 술후 각군의 굴절이상의 변화는 단안 외직근만 후전을 시행한 경우는 술후 1주에 180도 경선 의 굴절력이 감소되는 직난시로의 변화를(p<0.05), 단안 내직근만 절제를 시행한 경우는 술후 1주에 90도 경선의 굴절력이 감소되는 도난시로의 변화를 가져다 주었고(p<0.05), 외직근 후전 과 내직근절제를 동시에 시행한 경우는 두가지 수술방법의 혼합으로 통계학적으로 유의한 변화 는 볼 수 없었다. 시간에 따른 굴절력의 변화는 술후 4개월에 단안 내직근만 절제를 시행한 군 의 90도 경선 굴절력을 제외하고는 각군에서 점차로 술전상태로 회복되었다. 따라서 사시수술 후 굴절력의 변화는 단안 내직근만 절제를 시행한 경우와 단안 외직근만 후전을 시행한 경우 180도 경선에서 술후 1주에 각각 각막의 Steep와 Flat 함을 보였고 후전과 절제를 동시에 시 행한 경우는 두가지 수술방법의 혼합으로 일관적인 변화는 볼 수 없었다.
