일차 생체 신이식에서 HLA 공유정도가 이식신의 생존에 미치는 영향

Abstract

There still is debates on the etfect and the value of HLA matching on graft survival in clinical renal transplantation. A few reports were presented in Korea about the effect, where over 95% of kidneys have been taken from living donor, and the results are inconclusive. So, we analysed 329 cyclosporine treated first living donor renal transplantation performed in Keimyung University Dongsan medical center, from Nov. 1982 to Feb. 1996 to determine the effect of HLA matching on graft outcome. Significant survival benefits were observed at 5 years between HLA-B 0 mismatch(MM) and more than 0 MM (89.73% vs 64.68, 60.26%, p<0.05), A+B 0 MM and more than 0 MM(94.28% vs 63.94, 63.43, 56.25%, p<0.05), B+DR 0 MM and 2, 3 MM(89.59% vs 61.82, 64.98%, p<0.05), and A+B+DR 0 MM and 3, 4 MM(92.57% vs 56.82, 63.19% p<0.05). But HLA-A or DR mismatch alone seemed to have no effect on graft survival. Our results showed some beneficial effect of HLA match on renal graft survival but yet inconclusive due to small size of the number.

1982년 11월부터 1996년 2월까지 계명대학교 의과대학 신이식팀에서 시행한 350예의 신이식환자중 에서 cyclosporine을 사용한 일차 생체 신이식 329예를 대상으로 HLA 공유정도가 이식신의 생존에 미 치는 영향을 보고자 실시한 본 연구에서 아래와 같은 결과를 얻었다. 생체 신이식 329예의 추적기간은 최소 6개월에서 최고 165개월이었으며, 이식신의 최고 생존기간은 140개월이었다. HLA-A+B 부적합 항원수에 따른 5 년 생존율은 0개(n=46)의 경우 94.28%, 1개(n=61)의 경우 63.94%，2개(n=154)의 경우 63.43%, 3개(n=57) 의경우 56.25%로서 0 mismatch 군과 나머지 군사이 에는 통계적 유의성이 있있다(p<0.05). 그리고 B 와 DR 전체에 대한 부적합 항원 숫자별로는 0개(n=36), 1개(n=23), 2개(n=77), 3개(n=123), 4개(n=55), 5개 (n=12) 의 경우가 각각 92.57%, 62.70%, 72.79%, 56.82%, 63.19%, 91.67%로서 부적합 항원이 하나도 없는 0 MM 군과 3개 이상의 부적합 항원군사이에는 통계 적 유의성(P<0.05)을 나타내었다. 그러나 HLA-A, B, DR 등 각각의 locus항원의 부적합정도와 이식신생 존율간에는 HLA-B를 제외하고는 통계적 의의가 없 었다.
