Abstract
The purpose of this study was to verify the effect of the program by developing a High Intensity Focused Ultrasound (HIFU) training program using virtual reality (VR) and applying to patients with uterine benign tumors. The study design was a quasi-experimental design by a non-equivalent control group pretest-posttest design. The data collection was made in two hospitals for women in D metropolitan city between November 16, 2017 and May 20, 2018. The research subjects were 54 uterine benign tumor patients who were hospitalized to be treated with HIFU. They were divided into the experimental group (26 patients) and the control group (28 patients). In order to measure the level of patient uncertainty, the MUIS (The Mishel Uncertainty in Illness Scale questions, which were originally developed by Mishel (1988) and translated by Jung et al (2005) were used. For measuring anxiety level, the tool by Spielberger (1972), which was translated into Korean by Kim & Shin (1978) was used. The pain level was measured using the NRS (Numerical Pain Scale) developed by Sartain & Barry (1999). To measure patients satisfaction, the tool by Oh (2012), which was modified and supplemented for the research purpose by the validity verification by the experts concerned, was used. In addition, the presence questionnaire by Witmer & Singer (1988), which was modified and supplemented by Lee (2007) was used for this study to verify the presence of the educational program using virtual reality. For data analysis, descriptive statistics, a pretest of homogeneity, independent t-test, and repeated measures ANOVA were used using the SPSS window IBM 21.0 program. The results of study were as follows: In the experimental group who was educated by the virtual reality (VR) program before the HIFU treatment, uncertainty (p<.001) and anxiety (p<.001) were significantly lower than the control group and patient satisfaction (p<.001) was significantly higher than the control group. The average score of presence questionnaire was 165.50 ± 9.50 in the experimental group, which was a proper level. Therefore, this study shows that the virtual reality (VR) education program was verified as an effective nursing intervention with the improved patient satisfaction by making patients experience situations in the virtual reality that could be hard to experience in reality and reducing the feeling of uncertainty and anxiety before the HIFU treatment.
본 연구는 자궁양성종양 환자 대상의 가상현실을 이용한 고강도 초음파치료(HIFU) 교육프로그램 개발하고 적용하여 효과를 검정하기 위한 목적으로 실시되었다. 연구설계는 비동등성 대조군 전후 설계를 이용한 유사실험 연구이다. 2017년 11월 16일부터 2018년 3월 20일까지 D광역시의 2개의 여성전문병원에서 수행되었고, 고강도 초음파치료를 시행하기 위해 입원한 자궁양성종양 환자 대상으로 하였으며, 실험군 26명과 대조군 28명을 최종 분석대상으로 하였다. 효과 검증을 위해 Mishel (1988)이 개발한 도구를 정재원 등(2005)이 번역한 것으로 불확실성을 측정하고, Spielberger (1972)의 도구를 김정택과 신동균(1978)이 한국인에 맞게 표준화한 불안도구로 측정하였다. Sartain와 Barry (1999)에 의해 개발된 숫자통증척도(NRS)를 이용하여 통증을 측정하였으며, 오유경(2012)의 간호만족도 도구를 본 연구자가 연구목적에 맞게 전문가의 타당도 검증을 통하여 수정, 보완하여 측정하였다. 또한 Witmer와 Singer (1988)에 의해 개발된 실재감 도구를 이형래(2007)가 수정 보완한 것으로 본 연구의 가상현실을 이용한 교육프로그램의 실재감 검증을 위해 이용하였다. 자료 분석은 서술통계와 사전 동질성 검정, Independent t-test, Repeated measure ANOVA를 이용하였다. 본 연구에 대한 결과는 다음과 같다. 자궁양성종양환자 대상의 가상현실을 이용한 고강도 초음파치료 교육프로그램은 실험군이 대조군보다 불확실성(p<.001), 불안(p<.001)이 유의하게 낮게 측정되었으며, 간호만족도(p<.001)는 유의하게 높게 측정되었다. 가상현실 교육프로그램의 실재감 측정을 위하여 실험군의 실재감 점수를 분석한 결과 165.50±9.50로 적절하게 나타났다. 따라서 본 연구에서 가상현실 교육프로그램은 직접 경험하기 어려운 상황을 가상현실 속에서 경험학습을 함으로써 대상자의 흥미를 유발하게 하고, 동기부여를 유도함으로 교육의 집중도를 높여주어, 시술 전 불확실성, 불안을 감소시키고 시술 후 간호만족도를 높이는 효과적인 간호중재로 검증되었다.
