Abstract
The purpose of the study was to develop and examine the effects of a biofeedback-based sleep improvement (BBSI) program for Korean elderly female with overactive bladder syndrome (OAB).A non-equivalent control group pre-post-test design was used. Data were collected from May 1 to August 31 in 2018. There were 20 subjects in the experimental and 18 subjects in the control group. SPSS Statistics 20.0 was used to analyze the data.The chi-square test, Fisher’s exact test, and Mann-Whitney test were used to test homogeneity. The Wilcoxon and Mann-Whitney tests were used for hypothesis testing, and Cronbach's test was used to assess reliability. The results of the study were as follows:1. The first hypothesis, “symptoms of OAB will be different between the experimental and control groups”, was tested. After completion of the BBSI program, the symptom severity of OAD (U=115.50, p=.041) and frequency of nocturia (U=45.00, p<.001) were significantly different between groups. Therefore, the first hypothesis was supported.2. The second hypothesis, “sleep patterns will be different between the experimental and control groups” was tested. Sleep quality (U=85.00, p=.005), sleep satisfaction (U=101.00, p=.017), sleep efficacy (U=74.00, p=.002), wake time after sleep onset (U=35.00, p<.001), number of awakenings, and number of awakenings in the 3 hours after sleep onset were significantly different between groups. Therefore, the second hypothesis was supported.3. The third hypothesis, “there will be a difference in autonomic nervous system activity between groups” was tested. After completion of BBSI program, standard deviation of all normal R-R intervals (U=138.00, p=.219), total power (U=160.00, p=.559), low frequency (U=178.00, p=.953), high frequency ((U=179.00, p=.977) and LF/HF ratio (U=169.50, p=.759) were not significantly different between the experimental and control groups. Therefore, the third hypothesis was not supported.4. The fourth hypothesis, “amount of physical activity will be different between the experimental and control groups”, was tested. There was significant a difference in amount of physical activity between groups (U=66.00, p=.001). Therefore, the fourth hypothesis was supported.5. The fifth hypothesis “level of depression will be different between the experimental and control groups” was tested. There was significant a difference in level of depression between groups (U=64.00, p=.001). Therefore, the fifth hypothesis was supported.From the above results, the BBSI program based on the senescent sleep model is an effective intervention for improving sleep in Korean elderly women with OAB. Applying the program systematically could be helpful in improving the quality of life of elderly women with OAB.
본 연구는 과민성방광증후군 여성노인을 위한 바이오피드백 수면증진 프로그램을 개발하고 그 효과를 검증하기 위한 목적으로 실시되었다. 연구설계는 비동등성 대조군 전후 설계이다. 자료 수집기간은 2018년 5월 1일부터 2018년 8월 31일이었으며, 연구대상 실험군은 20명, 대조군은 18명이었다. 자료분석은 SPSS Statistics 20.0을 이용하여, 사전 동질성 검정은 Chi-square test, Fisher’s exact test 및 Mann-Whitney test로, 가설검정은 Wilcoxon test와 Mann-Whitney test로 분석하였고 신뢰도 검사는 Cronbach’s를 이용하였다. 연구결과는 다음과 같다. 1) 제 1가설: “실험군과 대조군은 과민성방광증후군 증상에 차이가 있을 것이다”를 검정한 결과, 프로그램 실시 후 실험군과 대조군의 과민성방광증후군 증상심각도(U=115.50, p=.041)와 야간뇨 횟수(U=70.00, p<.001)는 유의한 차이가 있었다. 따라서 제 1가설은 지지되었다. 2) 제 2가설: “실험군과 대조군은 수면에 차이가 있을 것이다”를 검정한 결과, 프로그램 실시 후 실험군과 대조군의 수면의 질(U=85.00, p=.005), 수면 만족도(U=101.00, p=.017), 수면효율(U=74.00, p=.002), 입면 후 각성시간(U=35.00, p<.001), 수면 중 각성횟수(U=56.50, p<.001) 및 입면 후 3시간 이내 각성횟수(U=108.50, p=.035)에 유의한 차이가 있었다. 따라서 가설 2는 지지되었다. 3) 제 3가설: ‘실험군과 대조군은 자율신경계 활성도에 차이가 있을 것이다”를 검정한 결과, 프로그램 실시 후 실험군과 대조군의 standard deviation of all normal R-R intervals (U=138.00, p=.219), total power (U=160.00, p=.559), low frequency (U=178.00, p=.953), high frequency ((U=179.00, p=.977)과 LF/HF ratio (U=169.50, p=.759)에는 유의한 차이가 없었다. 따라서 제 3가설은 기각되었다. 4) 제 4가설: “실험군과 대조군의 신체활동량에 차이가 있을 것이다”를 검정한 결과, 프로그램 실시 후 실험군과 대조군의 신체활동량에 유의한 차이가 있었다(U=66.00, p=.001). 따라서 제 4가설은 지지되었다. 5) 제 5가설: “실험군과 대조군의 우울 정도에 차이가 있을 것이다”를 검정한 결과, 프로그램 실시 후 실험군과 대조군의 우울 정도에 유의한 차이가 있었다(U=64.00, p=.001). 따라서 제 5가설은 지지되었다. 이상의 결과로, 노화수면모델에 기반을 둔 바이오피드백 수면증진 프로그램은 과민성방광증후군 여성노인의 수면을 증진시키는데 효과적인 중재로 검증되었다. 추후 체계적으로 과민성방광증후군 여성노인에게 확대·적용하여 삶의 질을 향상시키는데 도움이 될 수 있을 것이라 생각된다.
