양손 하악 견인이 수술 후 인후통에 미치는 영향

Abstract

Tracheal intubation using double lumen endotracheal tube (DLT) causes postoperative sore throat. Two handed jaw thrust (2JT) increases space in the laryngeal inlet. We evaluated the effect of 2JT on postoperative sore throat (POST) in patients requiring DLT intubation. One hundred six patients scheduled one lung ventilation were allocated to two groups. In the 2JT group, 2JT was applied while intubation and advancement of DLT. In the control group, conventional intubation without 2JT was done. Incidences of POST were compared. The overall incidence of POST was lower in the 2JT group. 2JT maneuver decreased POST in patients undergoing DLT insertion for one lung ventilation.

이중 관 기관 내 튜브(Double lumen tube, DLT)를 이용한 전신마취는 수술 후 인후통을 일으킨다. 양손 하악 견인(Two handed jaw thrust, 2JT)는 후두 내 공간을 증가시키는 효과가 있어, 이 연구에서 2JT가 DLT 삽관 시 수술 후 인후통에 미치는 영향을 평가하였다. 한쪽 폐 환기를 시행하는 총 106명의 환자를 대상으로, 삽관 시 2JT 수기를 하는 실험군과 시행하지 않는 대조군으로 나누었다. 수술 후 인후통을 평가하였다. 수술 후 인후통의 총 발생률은 2JT군에서 더 낮았다. 2JT는 한쪽 폐 환기를 위해 DLT가 필요한 환자에서 수술 후 인후통을 줄일 수 있다.
