요추부 경막외 주사시 경막외 압력 파형 변화의 분석

Abstract

Epidural injection of the lumbar region is one of the methods for treating neuropathy caused by lumbar stenosis or intervertebral disc prolapse. In this study, a pressure transducer was used to visualize the pressure change when the needle entered the epidural space. We tried to find a more useful method for the patient by comparing the changes in the visualized pressure waveform. 104 patients were divided into midline group (44 patients) and paramedian group (60 patients). There were two types of pressure waveforms. The waveforms were divided into abrupt pressure decrease (grade Ⅰ) and gradual pressure decrease (grade Ⅱ) and compared with various elements. There was no significant difference between abrupt pressure decrease and gradual pressure decrease according to age, gender, body mass index, and diagnosis. However, the method of epidural approach showed statistical significant differences. Abrupt pressure decrease was observed 5 times more in the paramedian approach than in the midline approach.

요추부 경막외 주사는 요척추관 협착증 혹은 추간판 탈출증으로 인한 신경병증을 치료하기 위한 방법 중 하나이다. 본 연구는 압력변환기를 사용하여 경막외 공간으로 바늘이 진입할 때에 압력 변화를 시각화 하였다. 시각화 된 압력 파형의 변화를 이에 기여하는 요소(나이, 성별, 체질량지수, 바늘의 접근 방식, 진단) 들과 비교하여 환자에게 더 유용한 방법을 찾고자 하였다. 최종적으로 104명의 환자를 대상으로 midline 그룹과 paramedian 그룹으로 나누어 연구를 진행하였다. 환자에게서 나타난 압력 파형의 형태는 2가지의 형태로 나타났으며, 본 연구에서는 급격한 압력 감소(grade Ⅰ)와 완만한 압력감소(grade Ⅱ)로 나누어 각 요소들과 비교하였다. 나이, 성별, 체질량지수, 진단에 따른 급격한 압력감소와 완만한 압력 감소 사이에 유의미한 차이는 보이지 않았다. 그러나 바늘의 접근 방법에서 유의미한 차이를 보였으며, 측면 접근법에서 정중앙 접근법에서 보다 급격한 압력의 감소가 5배정도 더 많이 관찰되었다.
