혈청 프로칼시토닌 수치를 통한 Clostridium difficile 감염의 Severity 예측도 평가

Abstract
Clostridium Difficle Infection (CDI) can appear clinically in asymptomatic carriers, from acute infections to fulminant infections. In CDI infected patients, if the severity can be predicted early and the treatment direction can be determined by serological examination, it will be of great help in improving the treatment and the patient's prognosis. From March 2013 to January 2019, 87 patients who were diagnosed with CDI at Keimyung University Dongsan Hospital inpatients and outpatients who had a serum Procalcitonin (PCT) test within 72 hours were analyzed. Receiver operating characteristics analysis was used to measure the accuracy and sensitivity of PCT and CRP. Statistical analysis was performed by dividing into 46 non-severe patients (group1) and 41 severe and fulminant patients (group 2) based on the severity of CDI infection. Serum PCT was found to be 0.59±1.11 ng/dL in group 1, 5.37±16.99 ng/dL in group 2, and CRP was 7.09±6.29 mg/L in group 1 and 11.95±10.19 mg/L in group 2. Both numbers increased statistically significantly in group 2. The cut-off value that distinguishes groups 1 and 2 identified by Youden method was 0.27 ng/dL in PCT (sensitivity 86.8%, specificity 64.3%, positive prediction rate 70.2%, negative prediction rate 84.8%), and 2.62 mg/L in CRP ( The sensitivity was 92.1%, specificity 40.5%, positive predictive rate 58.5%, negative predictive rate 85%). Through this study, it was confirmed that the serum PCT level within 72 hours of symptom onset was closely related in determining the severity of CDI, and it was confirmed that the accuracy was higher when the severity was determined using the serum PCT level of 0.27 ng/ml.The above results are considered to be of great clinical help in that the non-severe group and the severe, fulimnant group can be estimated at an early stage, and the treatment direction and prognosis can be predicted accordingly, and further follow-up studies are expected.
Clostridium Difficle Infection(CDI)은 임상적으로 무증상 보균자, 급성 감염부터 전격성 감염까지 다양하게 나타날 수 있다. CDI감염 환자에서 혈청학적 검사로 조기에 중증도를 예측하여 치료방향을 결정할 수 있다면 임상에서 치료 경과 및 환자의 예후를 호전시키는데 많은 도움이 될 것이다. 본 논문에서는 위막성 대장염의 중증도를 예측할 수 있는 PCT 절단값의 확인 및 보편적으로 사용되고 있는 CRP와의 민감도, 특이도를 비교하였다. 2013년 3월부터 2019년 1월까지 계명대학교 동산 병원의 입원 및 외래에서 CDI를 진단받은 환자 중 72시간 내에 혈청 Procalcitonin (PCT)검사가 시행된 87명의 환자를 대상으로 수신기 작동 특성 분석(Receiver operating characteristics analysis)을 사용하여 PCT와 CRP의 정확도와 민감도를 측정하였다. 통계 분석은 CDI 감염중증도를 기준으로 non severe한 환자 46명(그룹1), severe 및 fulminant 한 환자 41명(그룹2)로 나누어 시행하였다. 혈청 PCT는 그룹 1에서 0.59±1.11 ng/dL, 그룹 2에서 5.37±16.99 ng/dL로 확인되었고 CRP는 그룹 1에서 7.09±6.29 mg/L, 그룹 2에서 11.95±10.19 mg/L 로 확인되었다. 두 수치 모두 그룹2에서 통계적으로 유의하게 증가하였다. Youden 방벙으로 확인한 그룹1과2를 구별하는 cut-off value는 PCT에서 0.27 ng/dL (민감도 86.8%, 특이도 64.3%, 양성 예측률 70.2 %, 음성 예측률 84.8 %), CRP에서 2.62 mg/L (민감도 92.1%, 특이도 40.5%, 양성 예측률 58.5 %, 음성 예측률 85 %)로 확인되었다. 본 연구를 통하여 증상 발현 72시간 내의 혈청 PCT 수치는 CDI의 중증도를 판가름 하는데 밀접한 관련성이 있음을 확인할 수 있었으며 혈청 PCT 수치 0.27ng/ml를 기준점으로 중증도를 판별할 시에 더 정확도가 높음을 확인하였다. 상기 결과는 조기에 Non-severe군과 severe, fulimnant군을 가늠하고, 이에 따라 치료방향 및 예후를 예측할 수 있다는 점에서 임상적으로 큰 도움이 될 것으로 사료되며 추가적인 후속적 연구가 필요할 것으로 보인다.
