모임과다 내사시에서 근사착술을 이용한 내직근후전

Abstract

To investigate whether the slanted medial rectus recession can correct excess esotropia at near without overcorrection at distance and reduce the distance-near deviation difference, we examined 11 convergence excess esotropia patients who had deviation at near exceeding distance esotropia by 15PD or more, and underwent this operation. They composed of 7 partially accommodative esotropia with high AC/A ratio and 4 nonaccommodative convergence excess esotropia. The surgical procedure consisted of bilateral, symmetrical slanted recession of the medial rectus muscle. The amount of recession of the upper and lower margins were calculated so as to correct the esotropia with correction at distance, and near respectively. The preoperative mean near and distance deviation were 33.8PD and 15PD, and each decreased to 9.2PD and 4.3PD postoperatively. The preoperative mean distance-near deviation difference were 18.8PD, and decreased to 5.4PD postoperatively. The 3 patients decreased over 15PD in the distance-near deviation difference. The 9 patients decreased over 10PD in the distance-near deviation difference. In conclusion, the surgical procedure, as bilateral slanted medial rectus recession, can correct excess esotropia at near without overcorrection at distance and reduce the distance-near deviation difference. Therefore, the bilateral slanted medial rectus recession may be used as the method of surgical treatment in convergence excess esotropia.

원거리 보다 근거리 편위각이 15프리즘디옵터 (PD) 이상 큰 모임과다 내사시 11명을 대상으로 근사착술을 이용한 양안 내직근 후전술을 시행하여 원거리 안위에서 과교정 없이 과다한 근거리 내사시를 교정하고，원-근거리간의 편위각 차이를 줄일 수 있는가를 알아 보고자 하였다. 모임과 다 내사시 환자의 구성은 고 AC/A비를 지닌 부분조절 내사시 7명과 비조절 모임과다 내사시 4명 이었다. 수술 방법은 양안 대칭 근사착술을 이용한 내직근 후전으로 수술양은 내직근의 윗변은 원 거리 편위각의 양 만큼, 아랫변은 근거리 편위각의 양 만큼으로 하였다. 근거리와 원거리 편위각 은 전체 술전 평균 33.8PD와 15PD에서 술후 각각 9.2PD, 4.3PD로 감소되었고, 원-근거리 편 위각의 차이는 술전 평균 18.8PD에서 술후 5.4PD로 감소되었다. 원-근거리 편위각의 차이가 15PD 이상 감소한 경우가 3명, 10PD 이상 감소한 경우가 9명이었다. 합병증으로 원거리에서 과 교정된 경우와 사착술에 의한 희선편위는 볼 수 없었다. 이상으로 모임과다 내사시에서 근사착술 을 이용한 양안 내직근후전은 원거리 안위에서 과교정 없이 근거리 내사시를 교정할 수 있었고， 원-근거리간의 편위각 차이도 줄일 수 있었다. 그러므로 양안 근사착술을 이용한 내직근 후전은 모임과다 내사시의 수술방법으로 사용될 수 있으리라 사료된다.
