복막 투석 환자의 사회적 지지, 피로 및 우울이 삶의 질에 미치는 영향

Abstract

This study is a descriptive research study that analyzes the effects of social support, fatigue, and depression on the quality of life of peritoneal dialysis patients. A total of 130 patients receiving peritoneal dialysis were examined for this study—who were being treated in an artificial kidney room in Y University Hospital in D Metropolitan City. The data collection was carried out from June to August 2021. Regarding data analysis, the SPSS Windows program version 27.0 was used to conduct descriptive statistics, one-way ANOVA, t-test, and hierarchical multiple regression analysis. The data analysis found the scores of social support (2.87±0.86), fatigue (4.48±1.68), depression (13.41±12.26), and the quality of life (2.61±0.70). The study found a significant difference, regarding the quality of life, in the following different characteristics of patients receiving peritoneal dialysis. Age (F=3.60, p=0.03), the highest level of education attained (F=3.40, p=0.037), religion (t=4.26, p<0.001), family monthly average income (F=3.32, p=0.039), dialysis-related hospitalization experiences (F=11.46, p<0.001), and dialysis information source (F=10.12, p<0.001).

Based on the correlation between social support, fatigue, depression, and the quality of life analysis, there was a negative correlation between social support and fatigue (r=-.74, p<.001) and between social support and depression (r=-.72, p<.001). There was a positive correlation between social support and the quality of life (r=.82, p<.001). Also, fatigue and depression (r=.76, p<.001) showed a positive correlation, while there was a negative correlation between fatigue and the quality of life (r=-.75, p<.001) and between depression and the quality of life (r=-.71, p<.001).

With the explanatory power of the model being 41%, factors including social support, depression, and dialysis information source impacted the quality of life significantly. In particular, social support (β=0.52) showed the most significant effect on the quality of life.

Overall, developing a nursing intervention education program that improves social support and reduces depression is necessary, as social support has the most significant effect on the quality of life of patients receiving peritoneal dialysis. People in medical professions would need to look for signs of depression in advance to refer the patients to counselors based on their severity. Also, there is a need to check patients receiving lifelong peritoneal dialysis so that any signs of depression can be monitored thoroughly. Thus, there is a continuous need for future studies to improve the quality of life of patients receiving peritoneal dialysis.

본 연구는 복막 투석 환자의 사회적 지지, 피로 및 우울이 삶의 질에 미치는 영향을 분석한 서술적 조사연구이다. 연구대상은 D광역시 Y대학병원 인공신장실에서 치료 받고 있는 복막 투석 환자 130명을 대상으로 하였다. 자료수집은 2021년 6월부터 2021년 8월까지 실시하였다. 자료 분석은 SPSS 27.0 program을 이용하여 기술 통계, t-test, one-way ANOVA, 위계적 다중회귀분석으로 분석하였다.

복막 투석 환자의 사회적 지지, 피로, 우울 및 삶의 질 정도를 분석한 결과 사회적 지지 2.87±0.86점, 피로 4.48±1.68점, 우울 13.41±12.26점, 삶의 질 2.61±0.70점으로 나타났다. 복막 투석 환자의 특성에 따른 삶의 질 차이를 분석한 결과 연령(F=3.60, p=0.03), 최종 학력(F=3.40, p=0.037), 종교(t=4.26, p<0.001), 가족 월평균수입(F=3.32, p=0.039), 투석 관련 입원 경험(F=11.46, p<0.001), 투석 정보 제공원(F=10.12, p<0.001)은 삶의 질에 유의한 차이가 있는 것으로 나타났다. 복막 투석 환자의 사회적 지지, 피로, 우울 및 삶의 질 상관관계를 분석한 결과 사회적 지지와 피로(r=-.74, p<.001), 사회적 지지와 우울(r=-.72, p<.001)은 유의한 음의 상관관계를 보였으며, 사회적 지지와 삶의 질(r=.82, p<.001)과 유의한 양의 상관관계를 나타났다. 피로와 우울은 (r=.76, p<.001)과 유의한 양의 상관관계를 보이며, 피로는 삶의 질(r=-.75, p<.001), 우울과 삶의 질(r=-.71, p<.001)은 음의 상관관계로 나타났다.

복막 투석 환자의 사회적 지지, 우울 및 투석 정보 제공원은 삶의 질에 영향을 미치는 유의한 영향요인이었고 모형의 설명력은 41%이다. 그 중 사회적 지지(β=0.52)는 삶의 질에 가장 유의한 영향으로 나타났다.

본 연구결과를 종합하여 볼 때, 복막 투석 환자의 삶의 질에 가장 많은 영향을 미치는 사회적 지지를 향상시키고 우울을 감소시켜주는 간호 중재 교육프로그램 개발이 필요하다. 의료인은 우울을 조기 사정하여 정도 및 심각성에 따라 전문가와 상담을 연결하여 치료를 받을 수 있도록 돕고 일생동안 복막 투석을 해야 되는 투석 환자의 우울을 집중적으로 관리할 필요가 있다. 이에 복막 투석 환자의 삶의 질을 증진 시킬 수 있는 방법을 모색하는 연구가 지속적으로 필요하다.
