웃음요법이 요양병원 입원 노인의 우울과 수면에 미치는 효과

Abstract
The purpose of this study was to investigate the effect of laughter-therapy on depression and sleep quality with elderly patients in long-term care hospitals.
The research design of this study was the quasi-experimental design of pretest-posttest nonequivalent groups. A total of 37 subjects were selected to participate in this study: 19 were assigned to an experimental group (laughter-therapy group) and the other 18 to a control group. Data collection was conducted at two long-term care hospitals H and S in Daegu from August 2 to 28, 2015.
The laughter-therapy was given twice a week for 40 minutes for four weeks (eight sessions in total) to the experimental group. Usual nursing care according to the policy of the long-term care hospital was offered to the control group. In order to gauge the effect of laughter-therapy, a structured questionnaire was used to measure general characteristics, degree of depression and quality of sleep. The degree of depression was measured using the Korean version of Geriatric Depression Scale Short Form (GDSSF-K), which was originally developed by Yesavage and Sheikh (1986) and translated by Kee Back-suk (1996). Pittsburgh Sleep Quality Index (PSQI) Korean Version, originally developed by Buysse (1989) and translated by Choi (2012), was used to measure their sleep quality. Data analysis was conducted using the IBM SPSS Statistics 22.0 Program, and the real number, percentage, χ²-test, independent t-test and Mann-Whitney U test were used to analyze the data.
The findings of the study were as follows:
Hypothesis 1: “The depression score in elderly patients who have participated in a laughter-therapy will decrease compared to elderly patients in control group (no laughter-therapy treated)” was supported (t=-7.12, p<.001).
Hypothesis 2: “The sleep quality score in elderly patients who participated in laughter-therapy will decrease compared to elderly patients in a control group (no laughter-therapy treated)” was supported (Z=-4.16, p<.001).
Based on the findings of the study, laughter-therapy has positive effects on depression and improvement of sleep quality in elderly patients in long-term care hospitals. Thus, laughter-therapy can be provided to reduce depression and improve sleep quality for elderly patients in long-term care hospitals as one of the meaningful and effective nursing intervention programs.
본 연구는 웃음요법을 요양병원 입원 노인에게 적용하여 우울과 수면에 미치는 효과를 알아보기 위해 시행되었다.
연구설계는 비동등성 대조군 전후 설계의 유사 실험 연구이며, 자료수집 및 실험 처치는 2015년 8월 2일부터 2015년 8월 28일까지 4주간 대구시에 위치한 H 요양병원과 S 요양병원에 입원한 대상자 중 대상자 선정기준에 적합한 노인을 임의로 표출하여 최종분석자는 실험군 19명, 대조군 18명으로 총 37명을 대상으로 하였다. 실험은 매회 40분씩, 주 2회, 4주간 총 8회를 실시하였다.
연구도구는 구조화된 설문지를 이용하여 일반적인 특성 및 질병 관련 특성, 우울 정도, 수면의 질 정도를 측정하였고, 우울 정도는 Yesavage와 Sheikh (1986)가 개발한 Geriatric Depression Scale Short Form (GDSSF)를 Kee (1996)가 국문화한 Geriatric Depression Scale Short Form-Korea Version (GDSSF-K)를 사용하였다. 수면을 측정하기 위한 도구로는 Buysse 등(1989)이 개발한 Pittsburgh Sleep Quality Index (PSQI) 도구를 Choi 등(2012)이 국문화한 도구를 사용하였다. 자료분석방법은 IBM SPSS Statistics 22.0 프로그램을 이용하여 실수, 백분율, χ²-test, independent t-test, Mann-Whitney U test를 사용하였다.
본 연구의 결과를 요약하면 다음과 같다.
제1 가설 : ‘웃음요법에 참여한 요양병원 입원 노인은 웃음요법에 참여하지 않은 요양병원 입원 노인보다 우울 점수가 감소할 것이다’는 지지 되었다(t=-7.12, p<.001).
제2 가설 : ‘웃음요법에 참여한 요양병원 입원 노인은 웃음요법에 참여하지 않은 요양병원 입원 노인보다 수면의 질 점수가 감소할 것이다’는 지지 되었다(Z=-4.16, p<.001).
이상의 결과에서 요양병원 입원 노인을 위한 웃음요법은 우울과 수면의 질 개선에 효과가 있음을 알 수 있다. 따라서 웃음요법은 요양병원 입원 노인에게 우울과 수면의 질 개선을 위해 유용한 간호중재로 활용될 수 있을 것으로 사료된다.
