간호사-의사 협력에 대한 간호사 및 의사의 인식 : 질적 내용분석

Abstract

The purpose of this study was to explore nurses' and physicians' perceptions of meaning and influencing factors of nurse-physician collaboration, and important situations which that need nurses and physicians collaboration, and to propose useful data for promoting the collaboration. Focus group interviews and qualitative content analysis were used for this research. The participants were 10 nurses and 8 physicians working at an university hospital in a metropolitan city, and the data were collected from May 28th to June 28th 2014. The questions covered their perceptions of the meaning of nurse-physician collaboration, influencing factors and important situations. The interview for each focal group took about 100 to 120 minutes. The data were transcribed and analyzed according to Elo & Kyngäs's (2008) qualitative content analysis method. As a result, according to 3 themes that is meaning, influencing factors, and important situations in nurses and physicians collaboration, 7 categories, 16 subcategories and 40 concepts were extracted. In this study, participants perceived the significance of nurses-physicians collaboration as a companion relationship and professionals` essential components promoting seamless task processes and good outcomes for patients. Factors influencing the nurse-physicians collaboration were perceptions and attitudes towards each other, supportive work environment and professional competence and spirit. When patients or their families claim problems or shortage in health care service, the patient's health can worsen. Also problematic situations happen as a result of non-provision of proper care, so nurse-physicians collaboration was particularly needed. In conclusion, nurses-physicians collaboration was particularly required in time to provide the best health care service for patients and their families, and which is based on the attitude of understanding and respecting each other. To promote the relationship, holding of regular meetings, creating an efficient environment and preparing organizational strategies promoting the competence of members were needed. Therefore this study will provide some useful basic data in order to develop of various education programs for helping nurses and physicians collaborate.

본 연구는 간호사-의사 협력에 대한 간호사 및 의사의 인식을 간호사-의 사 협력의 의미, 영향 요인, 중요한 상황을 중심으로 기술하고 이해하여 그 들의 협력 증진에 유용한 자료를 제공하기 위해 실시되었다. 본 연구는 포커스그룹인터뷰와 질적 내용분석을 적용한 연구이다. 2014년 5월 28일부터 6월 28일까지 D시 K대학교병원에서 근무하고 있는 간호사 10명, 의사 8명의 연구 참여자로부터 자료를 수집하였다. 초점집단 면담에 는 이들이 인식하는 간호사-의사 협력의 의미, 영향 요인, 중요한 상황에 대한 질문이 사용되었다. 각 초점집단의 면접은 약 100분에서 120분 정도 소요되었다. 필사된 노트와 현장 노트로부터 수집된 질적 자료는 Elo와 Kyngäs (2008)의 질적 내용분석방법에 따라 분석하였다. 분석 결과 3개의 주제 즉 간호사-의사 협력의 의미, 영향 요인, 중요한 상황에 따라 7개의 범주, 16개의 하위범주, 40개의 개념이 도출되었다. 본 연구 결과 참여자들은 간호사-의사 협력이란 원활히 업무를 처리하고 동반 자적 관계를 통해 환자에게 좋은 결과가 돌아가고자 하는 전문직업인으로 서의 발로라고 인식하였다. 간호사-의사 협력 영향 요인은 서로에 대한 인 식과 태도, 지지적 업무환경, 전문직업적 역량 및 의식이었다. 간호사-의사 협력이 중요한 상황은 환자나 보호자가 의료서비스에 대한 부족이나 문제 를 제기할 때 혹은 환자의 건강상태가 악화되거나 진료나 처치가 적절히 이루어지지 않는 문제 상황에서 원활한 대처가 필요할 때였다. 결론적으로 간호사와 의사의 협력은 환자 및 보호자에게 최고의 간호 및 의료서비스를 제공하고자 할 때 필요하며 서로를 이해하고 존중하고자 하 는 태도를 바탕으로 하는 것이다. 이를 증진시키기 위해 정례화된 회합의 활성화와 효율적으로 업무를 수행할 수 있는 환경조성 및 전문직업인으로 서의 역량을 증진시킬 수 있는 조직적 전략이 필요하다. 이상의 결과를 토 대로 간호사와 의사가 협력하는데 도움이 되는 교육 및 증진프로그램 개발 에 유용한 기초자료를 제공할 것으로 기대된다.
