전래놀이프로그램이 조현병 환자의 정서행동과 주관적 안녕감에 미치는 효과

Abstract

The purpose of this study was to develop nursing interventions for patients with schizophrenia and to measure the effect of Korean Traditional Play program on emotional behavior and subjective sense of well-being in patients with schizophrenia. This study was a nonequivalent control group pre-post test design. Twelve people for the experimental group and twelve people for the control group were selected among men and women between 30 and 60 years old with schizophrenia and with degenerated emotional behavior according to subject selection criteria. In this study, Korean Traditional Play program was done for 6 weeks; pre-test, post-test and next 8 weeks test were performed on the experimental and control groups, and then a week after the program was performed, a total of 5 tests, once a week, were performed on the experimental group. The data were collected from November 14 in 2011 to March 15, 2012. For experimental tools, emotional behavior criterion developed by Lee, Woo-Kyung (2004) was used, and for subjective sense of well-being, KmSWN (Korean Modification of Subjective Well-Being Scale in Patients with Schizophrenia under Neuroleptics), which was developed by Naber (1994) and later modified by Yoon, Jin-Sang (1998), was used. For the collected data, the general characteristics of the experimental and control groups calculated error and percentage were done using SPSS 12.0 and the test of normality for research variables was analyzed with Shapiro-Wilk test. For homogeneity test, Chi-square test and independent t-test were performed. The test of research hypothesis was analyzed with Independent t-test and paired t-test. The results of this study are summarized as follows. 1. Participants in the experimental group showed a significant difference in emotional behavior scores than those in the control group (p<.001). 2. There were statistically significant differences in the emotional behavior scores of the experimental group between pre-test and five total tests of the middle (p<.001), post-test (p<.001), next 8 weeks (p=.002), between post-test and next 8 weeks test (p<.001). 3. There were statistically significant differences in the subjective sense of well-being scores of the experimental group between the pre-test, second test (p=.007) and fourth test (p=.039). As a result, Korean Traditional Play program could be regarded as meaningful as a mental rehabilitation program based on our own traditional culture and shown to have influence on the emotional behavior field.

본 연구는 전래놀이프로그램이 개방병동에 입원한 조현병환자의 정서행동과 주관적 안녕감에 미치는 효과를 측정함으로써, 병동 내 조현병 환자를 위한 간호 중재를 개발하는데 그 목적이 있다. 본 연구는 유사 실험연구의 비동등성 대조군 전후 설계로, 정서행동이 퇴행되어 있는 30세 이상 60세 미만의 남·여 조현병환자 중 대상자 선정기준에 의하여 실험군 12명, 대조군 12명을 임의 표출하였다. 본 연구에서 전래놀이프로그램을 6주간 실시하였으며, 실험군과 대조군에 사전 조사와 사후 조사, 추후 8주째 조사를 실시하였고, 실험군은 프로그램 시행 후 1주일에 한 번씩 총 5회의 조사를 더 실시하였다. 2011년 11월 14일부터 2012년 3월 15일까지 자료 수집을 하였다. 연구도구로는 이우경 등(2004)이 개발한 정서행동척도로 사용하였고, 주관적 안녕감척도는 KmSWN(Korean Modification of Subjective Well-Being Scale in Patients with Schizophrenia under Neuroleptics)로 Naber 등(1994)이 개발하고 윤진상 등(1998)이 수정한 도구를 사용하였다. 수집된 자료는 SPSS12.0/PC 프로그램을 이용하여 실험군과 대조군의 일반적 특성은 실수와 백분율을 산출하고 연구변수의 정규성 검정은 Shapiro-Wilk test로 분석하였다. 동질성 검정은 Chi-square test, Independent t-test를 실시하였고 연구가설에 대한 검증은 Independent t-test, paired t-test를 분석하였다. 본 연구의 결과를 요약하면 다음과 같다. 1. 전래놀이프로그램에 참여한 실험군은 대조군보다 정서행동 점수가 통계적으로 유의하게 상승하였다(p<.001). 2. 전래놀이프로그램에 참여한 실험군의 정서행동 점수는 사전과 총 5회의 중간 조사(p<.001), 사전과 사후(p<.001), 사전과 추후 8주째(p=.002), 사후와 추후 8주째(p<.001)에서 통계적으로 유의한 차이가 있었다. 3. 전래놀이프로그램에 참여한 실험군의 주관적 안녕감 점수는 사전과 2회(p=.007), 사전과 4회(p=.039)에는 통계적으로 유의한 차이가 있었다. 이 이상의 결과로 볼 때 전래놀이는 우리 고유의 문화를 바탕으로 한 중재프로그램으로서 의의를 두고 정서행동영역에서 효과가 있는 것으로 나타났다.
