호스피스간호수행과 관련요인 : 호스피스병동간호사 중심으로

Abstract

The purpose of this study is to describe the nurses' hospice care performance in a hospice ward and to investigate the related factors with hospice care performance. One hundred four nurses working at the hospice ward in P and D Cities participated in this study. Data collection was conducted from Feb. 1st 2012 to Mar. 31st 2012. Data analysis was conducted using descriptive statistics, t-test, one-way ANOVA, Scheffe test and stepwise multiple regression using SPSS/WIN 18.0 program. The results of the study are as followings: First, the score of hospice care performance is 3.25 points out of 4 points (perfect score). According to the domain of hospice care performance, the physical domain was the highest score, 3.37 points and social domain was the lowest score, 3.09 points. Second, hospice care performance was significantly different according to age, marital status, education, religion and importance of religion, job position, job satisfaction, and life satisfaction. Third, hospice care performance was significantly different according to the experience of death, having a license or a certification related to hospice care, and experiences related to patient’s death. Fourth, the factors related to the hospice care performance are life satisfaction, experiences in patient’s death, obtaining a license or a certification related to hospice care, and the importance of religion. Based on the findings of the results, the hospice nurses need to enforce their hospice care performances in the social domain. In order to improve the hospice care performances, the life satisfaction for the hospice nurse has to be improved. They need religious activities and more opportunities for hospice care education. as well as obtain a license or certification. In addition, when staff is being arranged at the hospice, the management should consider age, marital status, education, religion and personal characteristics. In conclusion, this study is suggesting basic data to improve the hospice care performance by researching the related factors of the performance. This study is different from the findings of the current studies of hospice care performance at a general ward and highlights the importane of research into hospice care performance for the hospice nurses.

본 연구는 호스피스간호사의 호스피스간호수행 정도와 관련요인을 파악함으로써 호스피스간호수행능력을 향상시킬 수 있는 방안을 마련하는데 필요한 기초자료를 제시하고자 시도된 서술적 조사연구이다. 연구대상은 D광역시, P광역시, P시에 소재하고 있는 병원의 호스피스 병동에서 근무하는 간호사 104명이었으며 자료 수집은 2012년 2월 1일부터 3월 31일까지 하였다. 연구도구는 호스피스간호수행 도구를 사용하였고, 수집된 자료는 SPSS/WIN 18.0 program을 이용하여 실수, 백분율, 평균, 표준편차, t-test, one-way ANOVA, Scheffe test, stepwise multiple regression으로 분석하였다. 본 연구 결과는 다음과 같다. 첫째, 대상자의 전체 호스피스간호수행은 4점 만점을 기준으로 평균평점 3.25점이었으며, 영역별 호스피스간호수행 정도는 신체적 영역(3.37점)이 가장 높았고, 사회적 영역(3.09점)이 가장 낮았다. 둘째, 일반적 특성에 따른 호스피스간호수행 정도 차이는 연령, 결혼상태, 최종학력, 종교, 종교의 중요도, 직위, 직무만족도, 삶의 만족도에서 유의한 차이가 있었다. 셋째, 임종관련 특성에 따른 호스피스간호수행 정도 차이는 임종관련 교육경험 호스피스 간호 관련 자격증 및 수료증 취득 여부, 담당환자의 죽음 경험횟수에서 유의한 차이가 있었다. 넷째, 대상자의 호스피스간호수행에 영향을 미치는 요인은 삶의 만족도, 담당환자의 죽음 경험횟수, 호스피스 간호 관련 자격증 및 수료증 취득 여부, 종교 중요도로 나타났고 가장 높은 영향 요인은 종교 중요도로 나타났으며, 이러한 영향요인들이 호스피스병동간호사의 호스피스간호수행을 29% 설명하였다. 이상의 결과를 종합해 볼 때 호스피스병동간호사의 사회적 영역의 호스피스간호수행을 보완하고 강화할 필요가 있으며, 호스피스간호사의 호스피스간호수행 능력을 향상시키기 위해 호스피스간호사들의 삶의 만족도를 향상시키고 종교생활 및 호스피스 간호 관련 교육의 기회를 제공함과 더불어 자격증을 취득할 수 있는 방안을 모색할 필요가 있다. 또한, 호스피스 병동의 인력배치 시 연령, 결혼상태, 최종학력, 종교, 임상경력 등의 개인적 특성을 고려한 병원차원의 인사관리가 필요하리라 본다. 결론적으로 본 연구는 호스피스간호수행에 미치는 영향을 파악함으로써 호스피스간호 수행 능력 향상에 필요한 기초자료를 제시하고, 일반병동간호사를 대상으로 한 호스피스간호수행 정도를 파악한 선행연구와는 달리 11기관의 병동형 호스피스에 근무하는 호스피스간호사만을 대상으로 호스피스간호수행 정도를 파악했다는 데 그 의의가 있다.
